AFFIDAVIT

I, IMELDA L. KEYES, of legal age, Filipino, widow, and a resident of Maya Extension,
San Gabriel, Tuguegarao City, 3500 Cagayan, PHILIPPINES, after having been duly sworn to in
accordance with law, do hereby depose and state:

That my maiden full name is IMELDA R. LEGASPI; my first name is not KENDRA, and the fact
of my birth was duly recorded before the Office of the Local Civil Registrar of Calbiga, Samar.
Copy of my Certificate of Live Birth is herein attached as ANNEX “A”;

That on November 16, 2006, I and AARON MICHAEL KEYES married each other before Hon.
Jose N. Binarao, Judge, Municipal Trial Court in Cities, Branch 3, Tuguegarao City, Cagayan. Copy
of our Marriage Certificate is herein attached as ANNEX “B”;

That since our marriage, we live as husband and wife at Paranaque, Manila, and I carried his name
KEYES;

That on May 21, 2010, my spouse Aaron Michael Keyes died at Manila Medical Center, Manila;

That after the death of my spouse, I resided at Maya Extension, San Gabriel, Tuguegarao City, 3500
Cagayan, not anymore at Paranaque;

That on August 26, 2010, I received a Letter dated August 9, 2010 from the Department of Veterans
Affairs, Manila, informing me that I am entitled to receive Veteran’s Benefit being the surviving
spouse of Aaron Michael Keyes;

That in the said Letter, however, my name was erroneously entered as KENDRA KEYES;

That the said error is clearly a clerical or typographical one because my name is IMELDA L.
KEYES, and not KENDRA KEYES;

That in all my transactions and dealings before and after I got married with Aaron, I have been
using IMELDA, not KENDRA, as my first name;

That I declare under the penalty of law that the names KENDRA KEYES (referred to in the Letter)
and IMELDA L. KEYES refer to one and the same person;

That I am executing this affidavit to attest to the truthfulness of the foregoing narration of facts and
for any legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affix my signature this 27th day of August 2010 at
Tuguegarao City, Cagayan.
