AN ORDER IMPOSING GRANULAR LOCKDOWN IN PUROK 6, BRGY.

WHEREAS, the World Health Organization (WHO) declared the spread of the COVID 19 virus
infection as pandemic;

WHEREAS, President Rodrigo R. Duterte issued Presidential Proclamation 922 declaring a State
of Public Health Emergency throughout the Philippines;

WHEREAS, Section 2 of Presidential Proclamation 922 enjoins all government agencies and
Local Government Units to render full assistance and cooperation and mobilize necessary
resources to undertake critical, urgent and appropriate response and measures in a timely
manner to curtail and eliminate the COVID 19 threat;

WHEREAS, Section 6(e) of R.A. 11332, otherwise known as Mandatory Reporting of Notifiable
Diseases and Health Events of Public Health Concerns Act, provides that the DOH and its local
counterparts have the statutory and regulatory authority to enforce, among others, rapid
containment, quarantine and isolation, disease prevention and control measures and response
activities as well for events of public health concerns;

WHEREAS Section 2 of Executive Order No. 112, Series of 2020, President Rodrigo Roa Duterte
authorized City Mayors to impose, lift or extend the ECQ of barangays;

WHEREAS, Section 16 of Republic Act No. 7160 also known as the Local Government Code
provides that within their respective territorial jurisdictions, local government units shall ensure
and promote the health and safety as well as the comfort and convenience of their inhabitants;

WHEREAS, on September 13, 2020, the City Health Office had confirmed that one member of a
family in Purok 6 ____________, this City, was tested positive of Covid19 and is now under
proper treatment, and isolated;

WHEREAS, it is urgently necessary that pre-emptive measures be immediately imposed in Purok

6 of Barangay Kinawitnon, specifically to the five (5) HOUSEHOLDS which were identified to
have close contact to the person tested positive of the said virus, to prevent further spread of
the disease. Thus, extreme travel restriction is imposed;

NOW THEREFORE, I, AL DAVID T. UY, City Mayor of the Island Garden City of Samal, by virtue of
the powers vested in me by law, do hereby order the following:

SECTION 1. LOCKDOWN. It is hereby declared that the five (5) households situated at Purok 6 of
Brgy. Kinawitnon, Babak District, be placed under GRANULAR LOCKDOWN.

SECTION 2. LOCKDOWN DURATION. The lockdown under this Order shall be from 6:00 PM of
September 13, 2020 until such lockdown is herein lifted by the Local Chief Executive.
SECTION 3. RESTRICTION. NO family member shall be permitted to GO IN and OUT of the
perimeter of their respective residences for whatever reason. In addition, NO person shall be
allowed to visit the aforementioned households or enter the perimeter of their residences
except with prior approval of the concerned authorities;

SECTION 4. PURPOSE OF THE LOCKDOWN. The lockdown shall be for the purpose of conducting
rapid mass testing, surveillance, risk assessment, contact tracing, disinfection and containment
of the spread of virus;

SECTION 5. IMPOSITION OF ECQ. During this period, the Enhanced Community Quarantine
(ECQ) guidelines shall be in effect.

SECTION 6. DEPLOYMENT OF SECURITY PERSONNEL. The Philippine National Police and other
security personnel in coordination with the barangay are hereby directed to cordon the
identified households of Purok 6, ___________ to ensure that NO person shall enter or exit the
area during lockdown.

SECTION 7. MASS RAPID DIAGNOSTIC TESTING. The City Health Office together with barangay
health workers and other authorized health agencies will conduct random rapid mass testing
and impose other health protocols for COVID 19 infection. Residents found positive with COVID
19 will be committed to the government isolation facility immediately;

SECTION 9. VIOLATION. Persons that violate this Order may be arrested.

SETION 10. REPEALING CLAUSE. All prior orders, issuances, rules and regulations which are
inconsistent herewith are deemed repealed or modified accordingly.

SECTION 11. EFFECTIVITY. This Executive Order shall take effect on 6:00 PM of September 13,
2020 and shall remain in force until lifted by the Local Chief Executive.
