I. General Information
PT. Bintang Maritim Internasional was instructed by PT. Nusapandi Maritim

Daniswara, Carina P&I Correspondents in Jakarta, Indonesia on December 15, 2020
to attend at Taboneo Anchorage in order to investigate the nature and extent of
damage due to Barge PST 315 towed by TB. Pancaran 311, Drifting and made
contact with Floating Loading Facility Habco Trans 8, at Taboneo Anchorage, South
Kalimantan on 12 December 2020
Information available from the Ship Particular which have received from Master TB.
Pancaran 311, I noted as follows:
- Barge PST 315 is Steel Barge, Indonesia Flag, registered in Batam, BKI Class
and Gross Tonnage is 4,699 tons with Nett Tonnage is 1410 tons. The length
of the Barge is 96.56 m, breadth 27.43 m, depth 3.75 m. The Barge built in
2014.
Owner of the barge is PT. Pancaran Samudera Transport.
- TB. Pancaran 311 is Steel Tug – Twin Screw Tug, Call sign is YDA-6841,
IMO no. is 8670538, Built in 2011, BKI Class, Indonesia Flag, registered in
Samarinda and Gross Tonnage is 207 tons with NT is 63 tons. The length of
the Tug is 26.60 m, breadth 8 m, depth 3.75 m. The Tug have Main Engines
2 x Caterpillar.
Owner of the Tug is PT. Pancaran Samudera Transport.

II. Survey
- On December 17, 2020, at 11.00 hours LT, we departed Jakarta to

Banjarmasin and arrived there at 14.00 hrs LT.
- When arrived Banjarmasin further we contacted Mr.Yan (PST Banjarmasin)

and Mr. Ipan (from Habco) to ask the weather condition at Taboneo Anchorage
area the location of FLF Habco Trans 8 .
- Information were received from them that weather condition still in bad
condition (raining with big waves and strong current condition)
- At 15.00 hrs LT, we met with Mr. Yan (PST Banjarmasin) to discuss schedule
on board FLF Habco Trans 8 and the exact coordinate position of TB.
Pancaran 311 / BG. PST 315
- On December 18,2020 at 07.00 hrs LT, we went to boat service area service
but when we was on board boat service, the boat service informed that
weather condition at Taboneo very bad condition and its risk if we go there.
- Therefore, we postponed traveling by boat service to Taboneo Anchorage

(FLF Habco Trans 8 and TB Pancaran 311/BG PST 315) and we also informed
to Mr. Bambang – PT. Nusapandi Maritim Daniswara.

- At 13.00 hours LT, we conducted short meeting with Mr. Yan – PST
Banjarmasin, to discuss weather condition at Banjarmasin and condition at
Taboneo.
- On December 19,2020 at 07.00 hrs LT, We arrived at boat service location

and after traveling around 2 hours, further at 09.10 hrs LT we on board FLF
Habco Trans 8 which found the anchorage position in 03°40,634’ S / 114°
30,019’ E . See photographs no.3 – no.4
- When we on board FLF Habco Trans 8, we met the Master and explained him
that we would like to inspect the damaged condition due to incident on
December 12, 2020

- During arrived location, we around inspection the vessel and found the sea
condition still in moderate with raining condition. See photographs no.5 - no.7
- At 09.30 hrs LT until 10.00 hrs LT, we conducted meeting with participants as
follows :

- In the meeting time, we interviewed the Master of FLF Habco Trans 8

regarding chronology of incident on 12 December 2020 and further we
instructed him to provide the documents or photographs related to the incident
- The documents were received from Master FLF Habco Trans 8 as follows :
• Ship’s Particular
• Ship Log Book
• Watch duty
• Drawing of General Arrangement
The documents of FLF Habco Trans 8 provided in Appendix “B”
- The Master of FLF Habco Trans 8 was informed regarding the vessel :
❖ FLF Habco Trans 8 is Floating Loading Facility for transshipment

service cargo coals at Taboneo anchorage area with the owner is PT.
Habco Primatama and their subsidiary in Banjarmasin is PT. Multi Guna
Maritim
❖ FLF Habco Trans 8 is ideal for gearless vessel loading and it has
features that an ordinary floating crane does not:
▪ Guaranteed daily gross loading rate 25,000 MT of coals
▪ Capability to separate unwanted materials and contaminations,
such as metals.
▪ Capability to filter coal sizes
▪ Specially designed for faster loading cycles
▪ Capability to take cargo samples automatically

- They currently operates 2 (two) Floating Loading Facility in South Kalimantan

with detail as follows:
❖ FLF Habco Trans 8 servicing in Taboneo Anchorage area
❖ FLF Parameshwara servicing in Bunati Anchorage area
- In time of incident, the master said that the vessel still in waiting for their
activities to transshipment cargo coals at Taboneo anchorage area.
- During investigation to the master, I also noted as follows:
• The Master informed that on December 05, 2020 at 17.50 hrs LT clear
drop anchor at Taboneo Anchorage position in 03° 40,634’ S / 114°
30,019’ E in safe condition.
• On December 12, 2020 19.20 LT, with 2nd Officer in duty, further he
saw from AIS that there was an object 0.4 NM nearing to the bow of the
ship, then the position was already on the bow and further 2nd Officer
immediately took action by calling on VHF Channel 12 and Channel 17
to avoid collision.
• He found the object above is TB. Pancaran 311 with position in tied up
to the starboard hull of Barge PST 315.
• The barge made collision to the Port Fwd Side and Port Beam - near
draft mark of the FLF Habco Trans 8.
• As a result of the impact, the port fwd side of the FLF Habco Trans 8
and port beam were dented with scratched condition

- We also received documents in related incident, such as Statement of Fact,

Letter to Owner of TB. Pancaran 311/BG. PST 315 and Estimation of Repair
Claim which issued by PT. Multi Guna Maritim, Owner of FLF Habco Trans 8
with nominal claims is IDR. 270.995.880,- . Provided in Appendix C
- The damaged condition at FLF Habco Trans 8 was found during inspection
together with Master of FLF Habco Trans 8, as follows :

- After completed inspection to the damaged area, subsequently at 12.00 hours

LT we left FLF Habco Trans 8 and continue inspection to TB. Pancaran 311 /
BG. PST 315 at another location of anchorage.
- At 12.25 hours LT we arrived anchorage location of TB Pancaran 311 / Barge

PST 315 and at 12.30 hours LT we on board the vessel TB. Pancaran 311

- After we have met to Master TB. Pancaran 311, at 12.40 hours LT until 13.10
hours LT, we conducted meeting to get information and chronology of incident
on December 12, 2020.
- In the meeting time, we interviewed the Master of TB Pancaran 311 regarding

chronology incident and further we also instructed him to provide the
documents related to the incident, as follows:
• Chronology Incident
• Ship’s Certificates (found not on board, kept in Harbour Master)
• Cargo Documents & Passage Plan
• Deck Log Book
• Tide Table on December 2020
• Stability Booklet & Bollard Pull Test (Found not on board, kept in ship
agent)
- The Master also informed that ship’s certificates of the vessels not on board
due to were kept by Harbour Master in Banjarmasin, South Kalimantan.
- During investigation to the master and with refer to the Statement of fact
issued by Master TB. Pancaran 311, I noted as follows:
• The Master informed that on December 11, 2020 at 1330 hours LT
arrival at Taboneo Anchorage area with position in 03° 40,913’ S / 114°
29,418’ E and further the Barge PST 315 dropped anchor with 3 (three)
shackles in water with the barge in safe condition with water depth at
anchorage area in about 9 – 10 M. Provided in Appendix D

• We also asked the anchor weight and got information from Master that
the weight of anchor of the barge PST 315 is 1290 kg.
• TB. Pancaran 311 / BG. PST 315 were anchorage at Taboneo for the
purpose to discharge her cargo coals 10,500 mt to MV IVS Okudogo
(Mother vessel). Cargo documents are provided in Appendix E
• On December 12, 2020 19.00 hours LT, during bad weather condition
with big waves and heavy rain including strong winds from West East,
suddenly BG. PST 315 drifting.
• Due to the above condition, at 19.30 hours LT, Master immediately
stand by Main Engine and immediately action to proceed heave up
anchor.
• Because of bad weather condition with strong winds caused TB.
Pancaran 311 couldn’t keep drifting the BG. PST 315 and caused the
starboard side of the barge PST 315 collided to the port fwd side and
the paint on the port beam of the FLF Habco Trans 8. The Chronology
incident and Statement of Fact provided in Appendix F
• After collided to FLF Habco Trans 8, at 23.30 hours LT, Master of TB.
Pancaran 311 decided to anchorage at more save anchorage area and
at 23.45 hours LT, BG. PST 315 was completed process anchorage in
position 03° 41,500’ S / 114° 32,095’ E.
Log Book of the vessel provided in Appendix G
• Further Master TB. Pancaran 311 called to FLF Habco Trans 8 on VHF
Ch.17 and Ch.72 but found not any respond.e
• Master also informed that no any damage to BG. PST 315 related to
the above incident and we also took the copy of Tide Table on
December 2020. Provided in Appendix H
• On December 14, 2020 19.20 hours LT, TB. Pancaran 311/Barge PST
315 completed discharge cargo coals to MV IVS Okudogo
- After completed investigation to TB. Pancaran 311, further at 14.00 hours LT,
we left TB. Pancaran 311 and arrive Banjarmasin at 16.30 hours LT
- On December 20,2020 at 09.00 hrs LT, we conducted meeting with Mr. Yan,
PST Banjarmasin to discuss the regarding documents which not on board.
And information from ship agent that he will send by email the remaining
documents.

III. Estimation of Cost Damage Repair
The estimate on Claim Cost or Cost of damage repair regarding with the above
incident will be reverting
IV. Surveyor’s Notes
Based on our WP investigation and visual inspection on board FLF Habco Trans 8
and Barge PST 315, we would comment as follows:
1. As the Master had many time anchorage the above area, he should
understand the condition of Taboneo anchorage area to avoid the barge
drifting in bad weather.
2. During we investigated to Master TB. Pancaran 311 and with refer to his
Statement Of Fact, we found that the Master too late action to standby Main
Engine to keep the Barge during bad weather condition.
Repair schedule of the FLF Habco Trans 8 is still unknown.
3. Remaining documents to be provide by Ship owner of TB. Pancaran 311/BG.
PST 315 such as ship’s certificates, bollard pull certificate, passage plan and
stability booklet.
This survey was carried out to the best of our knowledge and ability and reflect the findings
at the time and place of inspection only, but does not reflect the nature and condition of any
concealed hidden parts, which are not accessible for inspection and is issued without
prejudice to the liability and/or interest of any of the parties concerned.
