Social history:

Lives with parents.

Does not drink.

Smokes 10-15 cigarettes/day for 30 years

Past medical history:

Unremarkable

Objectives:

19/O1/18:

Diagnosed as a case of Anxiety disorder (panic attack, depression?)

23/9/18:

Admission to hospital following MVA (motor vehicle accident)

Head injury (frontal lobe brain)

CT - several cerebral contusion

Also two right leg fractures (and an oblique fracture of right femur).

30/10/18:

Discharge from hospital and rehabilitation centre admission

19/11/18:

Discharge from rehabilitation centre - home with community rehabilitation

02/12/2018:

Presented with visual disturbances:

Complained ‘blurred vision and double vision’


Thinks vision problem due to the anxiety

Headache regularly 2-3 times weekly.

Headache and anxiety - feels well by Panadol and Prozac

Eye test :

Vision- 6/9 (decreased)

Wear glasses

Pupillary movement: limited

Conjugate gaze

Pupillary appearance- normal

Pupillary reaction-intact

Diagnosis:

Diplopia(?Cranial nerve abnormality)

Plan: Referral to Neuro-ophthalmologist and review in two weeks times.
