Confined Space Entry Checklist

Section 1 – Pre-Entry Preparations (To be checked by the master or responsible officer)

Are entrants certified for confined space entry?
Has the space been segregated by blanking off, physically removing a spool-piece or
piping section or isolating all connecting pipelines?
Have piping, inert gas and ventilation systems been isolated?
Has the space been cleaned of sludge or any other debris?
Effective ventilation will be maintained continuously while the enclosed space is
occupied. Where possible 2 inlet ventilation fans should be used (1 being the backup in
case of the failure of 1) + 1 extract fan fitted with semi ridged trunking to the worksite
area for extraction of any stale air or work fumes such as smoke etc.
Note: In order to obtain a representative cross-section of the compartment’s atmosphere, samples
should be taken from several depths and through as many openings as possible. Ventilation should
be stopped for about 10 minutes before the pre-entry atmosphere tests are taken
Notify production department about using extra Plant - or Utility air for ventilation
Pre-entry atmosphere tests:
Readings Oxygen      % vol (21%)
Hydrocarbon      % LFL (Less than 1%)
Toxic Gases      ppm (specify gas & LEL)
Note: Tests for specific toxic contaminants, such as benzene, hydrogen sulphide, carbon monoxide
and mercaptans, should be undertaken depending on the nature of the previous contents of the
space
Have arrangements been made for frequent atmosphere checks to be made while the
space is occupied and after work breaks? At intervals of       minutes
Note: If sludge is present, more frequent checks are required
Are fixed lighting, such as low voltage string lights or air-turbo lights ready for extended
entry periods?
SCBA Escape sets to be carried by all persons entering the Confined Space
Have 2 approved self-contained, positive pressure breathing apparatus and resuscitation
equipment ready for use at the entrance to the space?
Is rescue harness, complete with lifeline, ready for immediate use at the entrance to the
space?
Is fully-charged safety torch ready for immediate use with the entry team & at the
entrance to the space?
A responsible fully trained member of the crew is in constant attendance outside the
enclosed space, in the immediate vicinity of the entrance and in direct contact with a
responsible officer. These persons should be trained in the actions to be taken in the
event of an emergency
Has the Officer of the Watch (bridge, engine room, cargo control room) been advised on
the planned entry?
Have lines of communications been clearly established and understood by all
concerned?
Are emergency and evacuation procedures established and understood?
Is there a system for recording who is in the space (Confined Space Entry Log
Book/Sheet)?
Is all equipment used, of an approved type?

Section 2 – Pre-Entry Checks (To be checked by the person authorised as leader of the
team entering the space)

Section 1 of this checklist has been completed fully.
The personal protective equipment to be used by people entering the space must be prescribed. The
following items should be considered:
Protective clothing including work clothing or protective suits, safety boots, safety
helmet, gloves, safety glasses are used.
For large spaces, or where climbing access will be undertaken, the wearing of
safety harnesses may also be appropriate
Approved safety torches
Approved UHF or VHF radio Channel:
Personal gas detector or an area gas detector and alarm
Emergency Escape Breathing Device(s)
I am aware that the space must be vacated immediately in the event of ventilation
failure or if atmosphere tests change from agreed safe criteria.
I am aware that the space must be vacated immediately in the event of a Facility
General or Abandonment Alarm
I have agreed the communication procedures.
I have agreed upon a reporting interval of       minutes.
Standby Man has been identified and is aware of all precautions and conditions
relating to this entry including actions to be taken under all circumstances
Emergency and evacuation procedures have been agreed and are understood by all
involved.
