AFFIDAVIT OF DESISTANCE

I, DIANNE CARLA SY VELEZ, of legal age, with address at c/o

Unit 2E New Domain Bldg., No. 19 Holy Spirit Drive, Don Antonio
Heights, Brgy. Holy Spirit Drive, Quezon City, hereby state under
oath that:

1. I am the complainant in NPS No. XV-03-INV-19G-05839,

entitled Dianne Carla Sy Velez vs. Arwin Doming Velez,
which is a charge for alleged commission of “Concubinage”
punishable under Art. 334 of the Revised Penal Code
pending before the Office of Quezon City Assistant City
Prosecutor Maria Cecilia Gertrudes R. Salvador;

2. After several conferences with the respondent, I hereby

declare that the complaint arose out of pure
misunderstanding and misapprehension of facts;

3. By reason of such misunderstanding and misapprehension, I

manifest that I now withdraw the complaint against the
respondent and that I am no longer interested and I hereby
desist in prosecuting the abovementioned complaint;

4. I hereby release the respondent from all criminal liabilities

and waive any right of action against the respondent in
connection to the above complaint;

5. I am executing this affidavit of desistance without force,

intimidation, undue influence or threats exerted upon me
and to the best of my knowledge and ability.

6. I fully understand the legal consequences of this affidavit of

desistance;

7. I attest to the truth of the foregoing statements and

respectfully prays that the aforementioned complaint
against the respondent be withdrawn and/or dismissed.
