ACCOMPLISHMENT REPORT
COVERING THE PERIOD OF JANUARY 1- 31, 2021
(Foyer De Charite Quarantine Facility - Mangato Road, Laoag City, 2900 Ilocos Norte)

1. Triaging, screening and performing interviews for incoming Person Under Quarantine (PUQ)
to determine all aspects of possible COVID – 19 infections (i.e. health history, travel history,
and other important information as needed) using the Health Declaration Form and Health
Information Form while maintaining and observing proper infection control.
2. Checking documents needed prior to the admission of incoming PUQ. Checking temperature
and disinfect the things brought in the facility before sending to room assignment.
3. Assisting the incoming PUQs to their designated room accompanied by the NOD and IW
using proper PPE. Enlighten the incoming PUQs regarding the guidelines, rules and
regulations to follow within the facility.
4. Perform routine procedures (body temperature monitoring and signs and symptoms
monitoring) and fill in PUQ’ charts.
5. Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating
equipment, sanitizing surfaces, preparing beds etc.)
6. Track and maintain medical supply inventory along with other staff members.
7. Respond to inquiries, concerns and complaints from patients and their parents or guardians.
8. Daily submission of SCS Data through Email to designated authorities, securing consent and
explained fully the procedure of RT-PCR Test to the PUQ’s, and assisted the residents and
the health care team during the entire procedure of RT-PCR testing with complete PPE worn.
Disinfection of the room and the equipment/items used afterwards.
