AFFIDAVIT OF DISCREPANCY

I, LINA M. LUCAS, Filipino, of legal age, married and a resident of Brgy. Calibungan,
Victoria, Tarlac after having been duly sworn to in accordance with law, do hereby depose and
say:

1. That I am married to my husband, MELCHOR D. LUCAS, on 8 May 2000 in San

Jose City, Nueva Ecija as evidenced by the attached Marriage Certificate;

2. That we begot a son named, JOMEL ANDREI M. LUCAS, on 14 March 2002;

3. That in the Certificate of Live Birth of our son, the date of our marriage indicated was
8 April 2000 instead of 8 May 2000;

4. That such discrepancy or clerical error was due to the inadvertence of the person who
encoded it;

5. That I am executing this affidavit to attest to the truth of the foregoing

statements and inform the proper authority concerned about said discrepancy and
for whatever legal purpose this affidavit may duly serve.

IN WITNESS whereof, I have hereunto set my hand this 23rd day of September 2020 in
