answer our short survey questions presented in this questionnaire. This survey is needed as
compliance for our survey report, and our survey title is, "Survey Report: The Perception of
Senior High School Students and Teachers on the K to 12 Program." The goal of this survey is to
find out, what is the perception, or what is it that the Senior High School students and teachers
think about of the K to 12 Program. You can rest assured that the information in this
questionnaire will respect your anonymity. The estimated allotted time to answer this
