1. WHY IS MY DATA BEING COLLECTED? WHAT DATA IS COLLECTED?

WHO IS RESPONSIBLE FOR MY DATA?

• In order to create an engagement agreement between you and AVASO,

AVASO HR needs to collect information about you as part of onboarding &
registration with AVASO.

• Your collected data can be used for all the assignments allocated by AVASO
to you under the initial assignment or any future assignment while you
remain contracted with AVASO. Your personal information, including the
results of any required drug or background screens, may also be shared with
our clients as necessary for providing our services. In some cases, this
information is used to provide access to their facilities and run secondary
background checks through other agencies in order to verifying your identity.

• The collected data may include but is not limited to:

o Identity data
o Contact information
o Correspondence and billing address
o VISA/right-to-work information
o Background check reports including Criminal record, Credit record,
Address Verification, Education Verification.
o Bank information for payroll or other payments (Not shared with clients)
o Any other information necessary to provide service to AVASO clients.

• Note that all data is collected and used with your consent and in compliance
with the in-country national and European data protection laws.

• AVASO HR is responsible for your data protection and you can contact
AVASO HR to ask any questions that you believe to be useful including
withdrawal of this consent, please ensure you receive an acknowledgement
for such email communication to AVASO HR in some cases emails get
blocked by filters.

2. HOW IS MY DATA STORED?

• Your data is stored in the form of a paper and/or electronic dossier under the
responsibility of AVASO HR, who takes all necessary measures to ensure
data protection & security.

• If your data must be shared, it will be shared using a secure electronic

information exchange system and/or, in the context of legal proceedings, by
means of the E-deposit/DPA electronic system or similarly secure context.

• Your data is stored for the period of your association with AVASO and held
for a maximum 10 years post-association, unless a request for offboarding &
withdrawal of this consent is discussed with AVASO HR.

3. WHO WILL HAVE ACCESS TO MY DATA?

• Only those who must have access to your data to perform work functions and
AVASO HR will have access to your data. This includes general corporate
communications and communications specific to your assigned
engagements, finance, your leadership, the clients you support, and HR.

• This consent form will also authorize AVASO HR to share the data in your
dossier with other stakeholders (such as customer, backend support team,
building management) who are related of the service provided by you present
in country or outside of the country or outside of EU zone.

4. WHAT ABOUT A DATABASE?

• Your full name, address, qualification, email address and phone number will
be saved on our Database, Cloud system, in order assign and close tasks
under your name

• Your other personal, professional and financial data will be stored in HRMS
database cloud system, in order to process onboarding, verification and
payments

5. WHAT ARE YOUR RIGHTS?

• You have the right to give or withdraw your consent to the processing and
exchange of your data.

• If you agree to give your data now and to have them processed in the context
of your dossier, you can always withdraw your agreement later. In this case,
your AVASO HR point of contact will explain to you how your data will be
deleted from your dossier. It is possible however that not all the information
can be removed.

• You have the right to be informed of the purposes for which your data will
be processed and of the persons who will have access to your data. Your
AVASO HR point of contact will tell you who will be involved with
handling your dossier and who will have access to your dossier.

• You have the right to ask what information concerning you has been
recorded and to request corrections if some information is not correct.

• You have the right to receive all the information about you that has been
recorded in a portable and readable format.

• AVASO HR is responsible for these data.

• AVASO HR has the duty to ensure that your data are handled securely and to
notify you if the security of your data is not respected.

• If you are concerned about the way in which your data are handled, you can
contact your AVASO HR point of contact,

THIS CONSENT FORM CAN BE USED TO PROCESS AND EXCHANGE DATA IN
THE CONTEXT OF HANDLING THE DOSSIER WITHIN OR OUTSIDE OF EU/EEA
ZONE, FOR THE FOLLOWING PURPOSES:

 Verification of identity, working rights & Visa validity, background check

verifications including education & previous employment checks
 Assignment of service tickets and follow-ups
 Arranging vetting interviews for technical or/and language skills required for services
 Sharing of technical and non-technical information in order to perform services
 Gain site access at client premises
 Provide backend support by technical support person within or outside country/EU
Zone.
 Payroll/Payments processing
 Request for quotes for your service

I hereby acknowledge and declare the following:

• I declare that I have read this document and that its content has been explained to me.
• I have had sufficient time to consider the act of entrusting my personal data to
AVASO
• I have been able to ask all the questions that I wished.
• I know that my personal data will be processed for the purposes listed above, which
have been explained to me by AVASO HR team, that the members of his/her firm
will be able to access this when it is necessary, and that my data can be exchanged
with other stakeholders, and I consent to this.
• I understand that in connection with my proposed work for AVASO, their clients may
require consent to view any background check report concerning my history that may
be conducted by AVASO’s providers, Sterling, GoodHire, or similar vendor. By
signing where indicated below, I authorize AVASO to disclose the background check
for the purpose described above.
• This consent is valid until I have withdrawn it formally in writing. I also understand
that I am not obliged to give this consent and that I can withdraw it in writing in
whole or in part at any time.
