AFFIDAVIT OF INDEMNITY

I, CHRISTINE DONATO SALVAN, of legal age, Filipino citizen, married and a resident of
Block K16, Lot 20, San Antonio de Padua II, City of Dasmariñas, Cavite, having duly sworn
to in accordance with law, hereby depose and state:

1. That I am one of the surviving heirs of Renato Q. Donato who died intestate on
12 August 2012 in M.V. Santiago Medical Center, Brgy. Langkaan, City of
Dasmariñas, Cavite;

2. That I have been authorized by virtue of Special Power of Attorney ratified on

August 12, 2012 before Atty. Joselita P. Macaldo;

3. That the decedent died intestate and left no outstanding debts or obligations;

4. That at the time of his death, the decedent left real properties covered and
embraced by Transfer Certificate of Title No. T-5896 and Transfer Certificate of
Title No. T-690281 both of the Registry of Deeds for Cavite;

5. That the real property covered and embraced by Transfer Certificate of Title No.
T-5896 is presently mortgaged from the National Home Mortgage Finance
Corporation (NHMFC);

6. Being one of the surviving legal heirs of the decedent, I hereby adjudicate unto
myself the above-mentioned property pursuant to Section 1, Rule 74 of the Rules
of Court and I request National Home Mortgage Finance Corporation (NHMFC)
for the cancellation and release of mortgage of the above-mentioned property
and I hereby bind to keep National Home Mortgage Finance Corporation, its
Officers and Employees, free and harmless from any claims and damages it may
incur that may be filed against them.

7. I hereby indemnify National Home Mortgage Finance Corporation (NHMFC)

and kept them indemnified against any liability, loss, claim, damage or action of
whatsoever nature that may arise in the future relative to the cancellation of
mortgage of the said property.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 3 rd day of December,
