I/We understand that under the provisions of the Agreement, we have designated certain deposit account/s with BDO (“Account/s”), wherein we are required to
maintain a specified minimum monthly average daily balance (“ADB”).
In this regard, as additional Accounts for purposes of determining ‘Company’s compliance with the required ADB under the Agreement. I/we hereby signify our
consent to the inclusion and use of my/our following described BDO deposit accounts:

of this agreement
 
This consent and waiver shall take effect on the date hereof, and shall continue to be effective, valid and binding upon us, unless and until withdrawn or cancelled by
me/us by written notice received by BDO at least five (5) banking days prior to the intended date of withdrawal or termination.

kind of restriction, we shall immediately notify BDO in writing upon our learning thereof.
We understand that this letter-request is subject to BDO’s approval, and that in case of approval, the implementation hereof shall become subject to the terms and
conditions of the Agreement and BDO’s existing ADB Compliance Policies.
