New students also need to provide a hard copy of their medical record from their previous school. Students who have not submitted proof of vaccination previously are required to do so. The forms should be submitted on time so that the school does not have to continuously follow up. An up-to-date medical file is important and will be checked by the Ministry of Health.

    Dear Parents,

    This is to inform you that as per instruction from the Ministry of Health and Prevention (MOHP),
    all students MUST submit the General Consent form and the Students medical History form to
    the school clinic as a yearly mandatory requirement.
    New students need to provide us with a HARD COPY of their medical record from their previous
    school.
    All students who have not submitted a vaccination copy previously are required to do so.

    Kindly download, fill in and sign the attached forms.

    Please note that the forms should be submitted by 6 December.

    Please be courteous and submit the documents so that we do not have to continuously follow-
    up. An up-to-date medical file is of utmost importance and will be questioned by the Ministry of
    Health if it is not submitted yearly.
