Prequalification Checklist Form 
Contractors are requested to complete this form and return it as part of their submission.  All  questions 
should be answered on this form or an exact duplicate thereof. UNOPS reserves the right to check other 
sources available to verify any information submitted in the pre‐qualification submission. If an Contractor 
knowingly  makes  a  misrepresentation,  or  an  omission  of  a  material  fact,  in  submitting  information  to 
UNOPS, such misrepresentation or omission may be sufficient ground for denying pre‐qualification to that 
Contractor, rescinding the Contractors’ pre‐qualification, rejecting a Contractors’ or rescinding an award 
of the Contract or terminating the Contract itself.
