SUBJECT : Submission of Five (5)-Year Hospital Development Plans

All DOH hospitals, medical centers, and sanitaria are hereby directed to submit their
respective Five (5)- Year Hospital Development Plans (HDPs) using the attached template. The
HDPs are expected to reflect their respective directions in the Philippine Health Facility
Development Plan, which serves as guide for the upgrading of health facilities throughout the
country towards achieving Universal Health Care, as well as guide in ensuring the rational
allocation of government investments in health care delivery.

Submit the HDP and all

necessary annex documents to
the Health Facility Development
Bureau (HFDB), or through email address doh.hfdb.rpmd@gmail.com, on or before February
28, 2020. For inquiries or concerns, please contact Mr. Roderick M. Napulan, Division Chief
