About this document

An important element of the Workplace Assessment process is the completion of the

Workplace Assessment Portfolio. The portfolio includes a range of written tasks and
activities that require you to reflect upon your experiences and learning within the
workplace.

Submission Instructions
To complete the activities and tasks outlined in the Workplace Assessment Portfolio, type
in the answers to each assessment question in this document. Guidelines on the number
of words for each question are provided. Once you have completed your assessment and
scanned the additional pieces of required evidence, save and upload it to your unit, and
then submit the portfolio.

Assessment File Naming Convention

Please ensure that you use the following file naming convention when you save your Word
document.
Your file should be named and saved to your computer’s hard drive using your student
number assessment number.doc.
For example, 12345678_21850a_01.docx or 12345678_21888a_01.doc.
Your student number allows your assessor to identify to whom the assessment belongs
and the assessment number indicates which assessment is being submitted.

Submitting Your Assessment

When you are ready to submit your assessment, upload the file in OpenSpace using the
Workplace Assessment Portfolio Upload link in the final Module of the course. The
Student Lounge provides a Quick Guide to Uploading Assessments if you need further
assistance. Uploading assessments in OpenSpace will enable Open Colleges to provide
you with the fastest feedback and grade on your assessment.
It is important that you keep a copy of all electronic and hardcopy assessments submitted
to Open Colleges.

you must also clearly identify:
 the equipment used – i.e. cloths, paper towels and disinfectants (type, action, dilution,
amount of time to be left in contact with the surface, etc.)
 hazards associated with the task – e.g. handling of hazardous substances, open cage
door, etc.
 the actions you take to eliminate or reduce the identified risks.

Activity 4: Observe and record animal behaviour

Record and submit a three- to five-minute video of the behaviour of a companion animal
within the facility. Try to choose the animal based on your elective choices – i.e. dog, cat,
rabbit or guinea pig, rat or mouse, and where possible a young, sick or injured animal.
Seek consent from your workplace supervisor before videoing the selected animal. From
your footage:
1. Identify normal behaviour exhibited by the animal.
2. Identify abnormal behavior exhibited by the animal. If the animal is not exhibiting
any abnormal behavior, identify behavior that would suggest an animal is unhappy,
stressed or bored.
When making your observations, consider whether the animal is eating and drinking
normally, toileting normally, is socially interactive and responsive to attention, interested in
surrounding, sleeping normally or excessively.
Record your observations below.

Activity 5: Monitor and record animal general health and wellbeing

Select two different species that you will be able to observe, monitor and evaluate over a
period of time, and on a number of occasions within the facility. Choose the species based
on your elective choices – i.e. two of the following: dog, cat, rabbit or guinea pig, rat or
mouse. Where possible, one should be a sick, young or injured animal.
Using appropriate anatomical and physiological terminology, complete the below table to
record the general health, wellbeing and condition of the selected animals – including
signs of illness or injury.
Gather the required information from your own observations and from workplace
documents such as Animal Health Records. You will also need to indicate if any of your
findings required reporting to your workplace supervisor.

Workplace leaders require strong communication skills. Identify an occasion you have
communicated to other staff. For example:
 To explain the day’s work plan
 To communicate a change to a policy or procedure
 To promote a new sustainability initiative
Reflect on the skills you used and complete the following table. If you are unable to do this

All organisations have internal and external networks that are important to the team and
business (for example, colleagues in other parts of the organisation, suppliers, customers,
dog trainers, council rangers, vet food suppliers, professional associations, local
government, council and so on).
In the table below identify one internal network and two external networks, why the
network is important to the organization and strategies you will or have used to build
relationships in these networks.

carry out. As part of this commentary you must also clearly identify:
 the equipment used – i.e. broom, mop, stiff brush and disinfectants (type, action,
dilution, amount of time to be left in contact with the surface, etc.)
 hazards associated with the task – e.g. slippery floors or an aggressive dog, and the
risks to self, the animal and any bystanders
 the actions you take to eliminate or reduce the identified risks
 the type of bedding chosen, reason for the choice and maintenance of it.
Use the Observation checklist below as a guide to the steps you must carry out. Your
assessor will use it to check you have carried out these steps in the correct order.
