Dear Madam,

In the interest of service, it is with utmost urgency that I am requesting, humbly, from
your good Office an assistance for my Covid-19 Swab Testing, to be made by the
Health Personnel of Barangay XXX Health Center, at their earliest convenience.

On the night of 24 July 2020, Friday, I had a fever and a mild sore throat, which are
symptoms of the infamous viral disease that had recently stolen away thousands of
lives from their loved ones in a span of only a few months – Covid-19. Fearing that I
might infect my friends and family as well as my fellow public servants, I am now taking
strict precautions by isolating myself from them. However, not quite certain as to I am
negative of the said virus, it is with a brave heart that I want to have a Swab Testing in
order to instil peace of mind, not only upon myself, but also to my family and
acquaintances.

Thus, if it is not too much to ask from your good Office, I am respectfully praying that I
may be assisted in the conduct of my Covid-19 Swab Testing by the Health Personnel
of Barangay XXX Health Center, or any health center that provides the same testing, at
their earliest convenience.
