INSTRUCTIONS

1 A monitoring mechanism shall be adopted by each Department/Branch/Office where regular submission of weekly accomplishment report/s during
the implementation of the alternative work arrangement shall be required to be submitted to the Division/Group/Sector Head concerned.

2 The ratee shall accomplish daily the Performance Monitoring Report (PMR) and it shall be monitored daily by his/her immediate supervisor. The
mode of accomplishment shall be through One Drive in order for employees to have access with the PMR whether working from home or office.
The PMR shall be shared by the Administrative Staff of the Department / Branch / Office. Please see further details under One Drive instructions.

3 Employees who are on strictly work-from-home arrangement but will report to the office regardless of hours rendered shall choose O (Office)
under ACTUAL AWA SCHEDULE for the particular day he/she went to the office.

4 For Quality/Effectiveness, Percentage (%) of Accomplishment shall be computed based on internal standards or quality table set in the unit. Please
see sample tables under the Accomplishment of the PMR-Quality/Effectiveness

5 For Efficiency, Percentage (%) of Accomplishment shall be computed based on the formula for Performance Standards (PS) or Efficiency. Please
see Accomplishment of the Performance Standards (PS) or Efficiency.

6 For Timeliness, Percentage (%) of Accomplishment shall be computed by the supervisor depending on the submitted commitment date and shall
provide basis of the computation upon request. Please see Accomplishment of the PMR - Timeliness.

7 For the % Accomplished Columns, employees shall indicate the actual percentage (cumulative) accomplished as of the date of submission of the
PMR.

8 Use the measure/s applicable to the Major Final Output.

The Quality/Effectiveness (Q) shall be used to record and measure the Ratee's frequency of errors/revisions for a Major Final Output (MFO) versus
the allowed frequency. The latter shall be based on appropriate sources (e.g. operations data and ISO Quality Management System objectives of
the Department/ Branch/Office).

ACCOMPLISHMENT OF THE PMR-Quality/Effectiveness - (Q)

ERROR FREQUENCY shall be computed by using the formula:

PERCENTAGE (%) OF ACCOMPLISHMENT shall be determined by the standards set in the Department/Branch/ Office's internal
Quality Table.

The Efficiency-Actual/Target (E-A/T) shall be used record and measure the Ratee's actual performance or output against set target per Major Final
Output (MFO) for a particular month/period.

The Timeliness (PMR T) shall be used to record and measure the Ratee's actual performance per Major Final Output (MFO) for a particular
month/period.

ACCOMPLISHMENT OF TIMELINESS - (T)

Percentage (%) of Accomplishment shall be computed depending on the date accomplished, whether or not the ratee met the target date. The
basis of computation shall be official and applicable records which should be made available upon request.

REMINDERS

1. Remarks column may pertain to critical incidents affecting the attainment of a particular assignment and shall be filled up by the Immediate
Supervisor, if needed.

2. The Office Head shall be responsible in keeping the PMR and shall ensure the availability of the said document upon request of authorized
individuals. In such cases or as needed, the PMR shall be printed and signed accordingly.

SHARING / ACCESING PMR IN ONE DRIVE

1. Log in to your Office 365 account and go to One Drive.


2. Create a folder in OneDrive and rename it following the format: PMR_Dept/Branch/Office.
Ex: PMR_PMERD

3. Click the folder and upload the PMR Template. Share the folder once uploaded.

4. Before entering email addresses, change the sharing option to ‘Specific people.’ Then tick ‘Allow editing’ box and click ‘Apply’ button.
5. Enter the email addresses of the people with whom you will share the file and click ‘Send’.
