X. Rationale of the Project (brief description of the situation):

One of our members noticed that there are children in Barangay Santa Cruz that
are out of school. This program will help them to learn and develop their skills and
potentials, also it can fulfill their desire to learn even though they don’t have enough
resources

XI. Objectives (General and Specific):


This project generally aims to teach and educate the out of school children of Santa
Cruz Rosario Batangas for free. Specifically, the program aims to realize the following
objectives:
1. To give the out of school children with necessary knowledge.
2. To improve their socializing skills and to become active.
3. To prioritize the heath of every other children.

XII. Description of the Project, Strategies and Methods (Activities / Schedule):


The group proposed the program H.E.L.P Program. This program will help out of
school children. This aims to realize the following objectives: To give the out of school
children with necessary knowledge and to improve their socializing skills and to become
active. Lastly, for them to give priority to the health of every other children.

XIII. Financial Plan:


Fund for the program will come from the solicitation and support by other
officials of the Barangay.
XIV. Functional Relationships with the Collaborating Agencies (Duties / Tasks of the
Cooperating Agencies):

The group will collaborate with the SK Officials, Barangay Health Workers of
Santa Cruz, even Barangay Officials. Thus, the agencies may extend their assistance by
only means they prefer whether financial or moral support.
The project leaders are expected to perform the following:
1. Coordinate with the SK Officials of the Barangay
2. Facilitate the entire program.
3. Monitor and evaluate the program.

The project coordinators are expected to perform the following:


1. Carry out the assigned duties and responsibilities related to the program.
2. Maintain an open communication with the beneficiaries and facilitators
connected.
3. Make all materials available

The participants are expected to perform the following:


1. Participate actively during the program.
2. Cooperate sincerely with the facilitators.
3. Work decisively to ensure success of the program.

XV. Monitoring and Evaluation Mechanics / Plan:


The said program will be facilitated by the group. There will be member who
shall help and function in different interrelated roles to ensure the proper conduct of the
project. There will be members of the group for information dissemination,
documentation, food, evaluation, materials and resources. The project leaders,
coordinators, shall coordinate with the collaborating agencies including Barangay
Officials of Santa Cruz, BHW, SK Officials and agriculture office of Rosario to provide
support.

XVI. Plans for Ensuring the Sustainability of the Project:


The group recognizes the needs and makes it goal functional. To ensure the
sustainability of the project, the group strives to continually provide efforts and learning
in utilizing the resources that they have. Sustainability program can be assured through
effective planning of different aspects including finance, resources and organization of
the project.
