AFFIDAVIT

I, _________________, of legal age, Filipino, and a resident of Davao City,

Philippines hereby depose and say, THAT:

1. I am the branch manager and the authorized representative of the private

complainant ___________________________. in the criminal case no. _____
_____ against the accused _______________;

2. I personally signed the affidavit-complaint in above cited case;

3. The private complainant, through my representation and the wife of the accused
______________ave entered into an amicable settlement, with respect to the civil
aspect of the case, hereto attached and marked as Annex “A”;

4. By reason of the above-stated settlement, the private complainant is no longer

interested in further prosecuting the same provisionally;

5. I am executing this affidavit to inform the authorities concerned of the foregoing

facts with the collateral purpose of requesting the provisional dismissal of the
above-stated case, subject to the completion and full satisfaction of the obligation
by the wife of the accused ____________;
