Weekly LEV Checklist

Why do we need to check our LEV systems on a weekly basis?

All local exhaust ventilation (LEV) systems must be visually checked at least once a week to ensure
that the system components remain in a satisfactory condition. This weekly check does not replace
the requirement to undertaken thorough examination and testing of the system at least once in a
period of fourteen months as outlined in Regulation 9 of COSHH.

The purpose of this form is to identify the condition the LEV system is in as a means of preventing long
term problems developing. To use this form it is advised that the inspector addresses each question
in turn and provides descriptive comments and details any remedial actions that need to take place.
This form should then be handed to the system log book owner / Maintenance Manager for the record
keeping and action planning.

Record Keeping

This form should be stored alongside the fourteen-month Thorough Examination and Test reports and
maintenance records held for this system either in a physical log book or electronic database system.
Records are to be kept for a minimum of five years.
