PERFORMANCE STANDARD: Conducts surveys, experiments or observations and gathers

information from surveys, experiments, or observations*

SITUATION: The second semester for school year 2020-2021 is about to start. The
superintendent of Schools Division of Laoag considers continuing online as one of the
modalities. In view of the superintendent’s plan, you will conduct a survey.

GOAL: The survey aims to update the Division’s current data regarding the availability of
gadget/s and internet connection of learners at home.

ROLE: You are a high school teacher who will conduct the survey to your class which
consists of 20 students. (Choose whether you will conduct the survey via Google Docs,
Messenger, SMS, or phone call.)

PRODUCT: Using your revised questionnaire from Week 5 in your survey, prepare a
summary of the results and findings.

AUDIENCE: The summary of the results and findings will be used as one of the bases of the
superintendent in coming up with a decision.

STANDARDS: Your summary of the results and findings will be assessed using the
following standards: completeness, organization, clarity, and mechanics.
