TWO DISINTERESTED PERSONS

We, ROEL A. ROSALES and ALVIN M. COSTALES, both of legal age, Filipino citizens with
residence address at Brgy 47 Bengcag, Laoag City, Ilocos Norte, after being duly sworn in accordance
with law, hereby depose and state that:

1. We personally know JOSIELYN JUAN DANAO.

2. JOSIELYN JUAN DANAO has been our neighbor in Brgy. 47, Bengcag, Laoag City,
Ilocos Norte since _____;

3. This affidavit is executed to attest to the truthfulness of JOSIELYN JUAN DANAO’s true
identity and date of birth;

4. We know for a fact that the person by the name of JOSIELYN JUAN DANAO has a birth
date of 16 February 1984, but the Birth Certificate issued by the National Statistics Office
does not reflect such information;

5. We further attest that the foregoing discrepancy may be due to a typographical error;

6. JOSIELYN JUAN DANAO has other valid identification cards which she can present to
further support the foregoing facts; and

7. This affidavit is executed to attest to the truth of the foregoing facts, inform the proper
agency and for all legal intents and purposes.
