1 Intended Use
Cato is intended for inhalation anaesthesia in semi-closed systems both for mechanical
ventilation and for manual ventilation and spontaneous breathing. The low flow technique
used in semi-closed systems allows for a reduced consumption of gas and anaesthetic
agents.
Cato can be used with all usual inhalation anaesthetics (Enflurane, Isoflurane, Halothane,
Sevoflurane and Desflurane) by changing the anaesthetic vaporizer. Explosive anaesthetics
such as ether, cyclopropane or similar must not be used.
Any use of the equipment requires full understanding and strict observation of the relevant
Instructions for Use. The equipment may only be used for the purposes specified herein.
1.1.2 Maintenance
The equipment must be inspected and serviced by experts at a regular 6-month interval (and
a record kept). We recommend obtaining a service contract with DrägerService. Repairs may
only be carried out by DrägerService. Use only original Dräger parts for maintenance.
Carry out regular safety checks on the equipment as specified in the certification required by
the MedGV regulations.
1.1.4 Accessories
Only use accessories listed in the certification required by the MedGV regulations. Other
accessories may only be used if they are certified as safe and suitable for use with the
equipment.
The equipment may only be used in rooms with mains supply installations complying with
national safety standards (such as VDE 0107 in Germany). Follow the regulations laid down
in DIN IEC 601 / VDE 0750 for electrically powered medical equipment.
The equipment has not been approved for use in explosion-hazard areas.
For electrical connection with equipment not listed in the instructions for use consult the
respective manufacturer and/or an expert.

The liability for the proper function of the equipment is irrevocably transferred to the owner or
the operator if the equipment is serviced or repaired by personnel not employed or
authorized by DrägerService and/or if the equipment is used in a manner not conforming to
its intended use.

Prior to use, test the unit according to the check list attached to the equipment.
(MedGV 6 Section 4).
Using the relevant Instructions for Use (for the unit and additional devices) adapt the check
list to the type and the configuration of the unit by deleting and/or adding items as
applicable.
The check list covers the tests to be carried out on Cato before each start-up.
Enter the type identification and the serial number of the unit. Tick off the results of the tests
on the ACTUAL column. Do not forget to enter the date and your signature.
If the equipment is in order, the test described will be completed successfully. Should the
sequence be changed, follow the individual instructions.

1 Basic Unit
Configuration / Variants
Cato is designed as a 3-gas unit (O2, N2O and AIR) and comes with ORC low flow and low
flow flowmeters.
The unit can be retrofitted with an optional compressed gas cylinder set. This set consists of
two 11-liter cylinders (O2 and N2O). The high-pressure reducers (200 bar/ 5 bar for O2 and
60 bar/ 5 bar for N2O are located immediately below the cylinders. The cylinder pressure
can be read on the manometer of the respective pressure reducer. The cylinders are
connected to the unit by high-pressure coiled tubes. O2 and N2O is supplied to the
mechanical gas dosage through non-return valves.
O2, N2O and AIR of 2.7 to 5.5 bar is supplied via three ISO connectors on the rear panel of
the unit. The central gas supply pressures are indicated on manometers which are located
behind the flowmeter block.
Additional pressure reducers decrease the O2 and N2O pressures to 3 bar and the AIR
pressure to 1.5 bar. The respective gas is delivered to the flowmeter block depending on

which operating mode has been selected. After flowing through the flowmeter block, the gas
mixture flows into the Vapor unit where it is enriched with Halothane, Enflurane, Isoflurane,
Sevoflurane or Desflurane. The fresh gas is then available at the fresh-gas outlet.

1.1 Integrated Pneumatics
The integrated pneumatics replaces the conventional mechanical gas dosage. It covers the
function of the following individual components:
• check valves;
• N2O shutoff;
• control valve;
• alarm valve;
• mean pressure reducer
selector switch for Divan gas supply and secretion suction system in the event of AIR failure
threshold value switch for the selector switch
Function
An aluminium block is divided into three levels. Each gas (O2, N2O or AIR) is assigned to a
specific level. The pneumatic connection of the individual elements is ensured by channels
within the block. This method saves approx. 50% of plug-in connections compared with a
mechanical gas dosage system. The valves are designed as compact components. Short
switching paths and lack of diaphragms reduce the rate of wear and increase reliability and
life of the assembly.

The alarm valve is an integral part of the O2 deficiency alarm. In the event of O2 failure the
whistle in the O2 deficiency alarm is activated.

When the gas supply is open the container is supplied with gas through the check valve. The
existing O2 pressure closes the alarm valve. If the O2 pressure drops below 1.7 bar the alarm
valve opens. The O2 stored in the container flows through the injector to the whistle. The
injector draws ambient air and extends the alarm time. The audible alarm sounds for approx.
7 secs.

O2 and N2O are present at a pressure of 3bar. The AIR pressure is reduced to 1.5 bar. The
3-bar pressure is necessary for the function of the ORC which is located after the control
valve (AIR) and the N2O shutoff.

The control valve and the N2O shutoff are supplied with pressure via the O2 line. The control
valve closes. AIR cannot be supplied. The N2O shutoff is opened by the existing pressure.
O2 and N2O are available at the dosage valves of the flowmeter block. The N2O shutoff has
two functions. First, it is used as a changeover valve. Second, if the O2 supply fails or if the
minimum operating pressure drops below approx. 2 bar the spring will close the valve and
interrupt the N2O dosage.

The control valve and the N2O shutoff are not triggered. Venting occurs via the gas selector.
The control valve opens in this position. The N2O shutoff closes and prevents a dosage of
N2O. O2 and AIR are available at the dosage valves of the flowmeter block.

The O2 and N2O flow is controlled by spindle valves on the flowmeter block. Restrictors
located at the inlet ports of the flowmeters generate a flow resistance. The resistance level
varies with the position of the spindle valve and the restrictor values. The resistance exert a
force on the diaphragm of the ORC. As a result thereof the O2 pressure opens the ORC
valve and the N2O pressure closes it. The restrictors and the spring force are designed such
that a minimum O2 concentration of 25% by vol. is ensured. The restrictors limit the maximum
O2 flow to approx. 9 L/min.

Bypass:
A minimum O2 flow of 0.3 to 0.8 L/min is required to open the N2O branch. Working in the
low-flow range is made more difficult by the opening behaviour of the ORC since mixed gas
dosage is not possible in the lower flow range. A non-adjustable restrictor of 0.8 L/min in the

N2O branch solves this problem. The safety function of the ORC is disabled in this flow
range.
• It makes sure that a vital oxygen concentration is present in the fresh gas supplied to the
patient.
• It closes the N2O branch in the event of oxygen shortage.

The spindle valves on the flowmeter block meter the O2 and N2O. Resistors located in front
of the flowmeters’ inlet ports in the S-ORC generate a backpressure while the gas flows. The
backpressure level varies with the opening degree of the spindle valves and the value of the
resistors. The backpressure exerts a pressure on the diaphragm of the S-ORC causing the
O2 pressure to open the S-ORC valve and the N2O pressure to close the S-ORC valve. The
resistors and the spring energy are dimensioned such that a minimum concentration of 21%
by vol. O2 is ensured. The resistors limit the maximum O2 flow to approx. 9 L/min.

• Adjust the spindles (M 30306) for O2 and N2O such that they protrude about 1 mm from
the valve element M 30307.
• Set the distance between the top edge of the valve element (M 32941) and the
enclosure to about 10 mm.
Given a supply pressure of 5 ±0.1 bar and a supply rate of 20 ±1 L/min, the pressure
regulators for the N2O and O2 supply are adjusted dynamically to the following
backpressures:
The leak test must be carried out with the S-ORC valve in place.
• Fine-adjust the S-ORC on the N2O side using the spindle M 30306.
• Adjust the concentration to 25% by vol. O2,with an O2 flow of 1 L/min. The spindle
M 30306 on the O2 side remains unchanged.
• If the concentrations for the other O2 flows are not within tolerance, connect a
manometer to the inlet port of the O2 side of the S-ORC (E) and check whether the O2
control pressure is at an O2 flow of 200 mL/min is 25 +1 mbar.
• If the O2 control pressure is not 25 +1 mbar, adjust the value with the spindle M 30306
on the O2 side.
• After adjusting the O2 control pressure, check the concentration again at 1 L/min O2. If
necessary, correct the value with the N2O spindle.

After adjusting/re-adjusting the S-ORC, the concentration test ist done with an O2 analyzer.
If the adjustments on the S-ORC have not been changed, the check can be done with the
test certificate. In this case, the flow values can be used for testing. When replacing the
entire S-ORC, testing according to the test certificate is also satisfactory.
The secretion suction system (including the secretion containers) is installed on one side of
the trolley. During normal operation the secretion suction system is powered by an adjustable
compressed-air ejector. In the event of AIR failure the ejector can be powered with O2.

The power supply unit provides the dc voltages required for the Divan and supplies the
PM 8050cd with the mains voltage via an isolating transformer. The isolating transformer is
connected to the power switch.

The mains voltage flows to the non-switched socket D and, through fuses Si1 and Si2, to the
central power switch on the front panel of Cato. The contactor K2 remains in the rest position
(contacts K2 are open) as long as the power switch is open. This means, the whole power
supply unit is de-energized (except for the above mentioned socket D).
The contactor K2 is supplied with mains voltage via the autotransformer and closes. The
isolating transformer T1 is supplied with voltage via the closed contacts of K2, Si3, Si4 and
the mains voltage selector switch. The mains voltage selector switch has four levels. Three
levels switch the pick-offs of the isolating transformer according to the position of the switch
and the mains voltage applied. The fourth level is used to supply the autotransformer T2 with
the voltage required to energize the contactor K2.
The electrically isolated output of the isolating transformer is protected by the fuse Si5. It
supplies the PM8050 cd via X2 and the power supply PCB (Divan) via X5. The output X6 of
the voltage supply provides Divan with the following voltages:

Powerfail:
The Powerfail signal is made available by optocouplers and open collector outputs.
The ventilator in the Cato system is an electronically controlled piston cylinder system (13)
which is driven by an electric motor. The ventilator has a compact breathing system (1-9)
which can be removed separately.

The electronics converts the values set by the operator into control signals for the drive motor
and the control solenoid valves. The control valve exerts a pressure (control pressure) on the
pneumatic diaphragm valves in the compact breathing system in order to generate the
desired ventilation mode. The respective position of the piston in the cylinder is determined
by an incremental encoder and the value is channeled back to the control unit.

The function of the piston cylinder unit is to deliver the fresh gas supplied from the flowmeter
block and stored in the breathing bag to the patient at a defined volume, pressure and speed.
During expiration the piston fills with the gas expired by the patient and with the fresh gas
stored in the breathing bag. During inspiration, this piston volume is administered to the
patient by an accurate stroke. Before reaching the patient it flows through a CO2 absorber.
To ensure this gas flow, the control system switches the valves of the breathing system
accordingly.
The most important elements of the piston cylinder unit are a piston which is moved by a
drive unit located in the rear part of the ventilator and a double roller diaphragm which is filled
with gas during operation and which functions as a sliding bearing. The double roller
diaphragm moves the piston along the inside walls of the cylinder. The front cylinder part
separates the ambient atmosphere from the respiratory gas of the patient. Several interfaces
are located on the underside facing the ventilator. The airway pressure is measured via a
pressure measurement connection (B1) and a pressure sensor. The pressure connection
(C1) provides the double roller diaphragm with the required positive pressure of approx.

160 mbar. The patient gas is supplied to the compact breathing system via a large lateral
outlet port.

The piston cylinder unit should be checked and serviced at regular intervals since the roller
diaphragm may become leaky as a result of ageing and frequent sterilization. The steps to
replace the roller diaphragm half on the patient side are as follows:
Step 1:
Remove the piston cylinder unit from the ventilator while in the standby mode.
Step 2:
Release the piston cylinder unit, pull straight up and remove.
Step 3:
Loosen the 2 quick-release locks (1) with a 6-mm wrench and remove the cylinder head (2).
Step 4:
Grasp the piston and unscrew the central piston screw using the 6-mm wrench and remove
the upper part of the piston (3).
Step 5:
Replace the roller diaphragm half (4) on the patient side against a new one. To ensure a tight
seal, the embossed manufacturing clock must point towards the upper part of the piston (3).
Step 6:
Use the reverse order for reassembly. Make sure neither kinks nor folds are formed during
installation of the roller diaphragm. The complete roller diaphragm must form a typical U-fold
at both ends.
Step 7:
Carry out the Cato power-on test to check the function and the tightness of the seal of the
roller diaphragm. The roller diaphragm must unroll without forming folds.
Note:
It is not advisable that the roller diaphragm half on the equipment side, ie. the non-
contaminated part, be replaced by the user. It should rather be replaced by DrägerService
since its safe function can only be ensured if various screw locking agents and special tools
(tightening torques) are used.

The fan (2) and the drive unit (1) are located at the rear part of the ventilator. To access the
drive unit for repair purposes, it is necessary to remove the complete ventilator from the Cato
basic unit.

The compact breathing system consists of a control valve plate (1) with the diaphragm valves
which are pneumatically triggered by the solenoid valves. The inspiratory and the expiratory
valves (7) responsible for the respiratory phase and the valve craters are located in the
respiratory valve base (2). The APL valve (Airway Pressure Limiting valve) (8) for pressure
control of the airway pressure during manual ventilation is located in the respiratory gas
branch. A check valve (3) and an emergency ventilation valve (4) are integrated in the airway.
The flow sensor element (9) of the PM 8050 cd is found at the connection of respiratory
hoses (12). The fresh gas from the flowmeter block flows into the breathing system via the
fresh gas terminal (15) and is stored in the breathing bag (16). In the inspiratory phase, it is
administered to the patient through the inspiratory valve (7) by a piston stroke. In the
expiratory phase, the gas flows through the expiratory valve (7) and the absorber element (11)
into the CO2 absorber container (10). The absorber element makes sure the expiratory gas
flows evenly through the entire surface of the soda lime in absorber.

Legend:
The two large lip seals on the underside of the respiratory gas base to the absorber element
(11) and to the absorber container (10) should be regularly checked for contamination or
damage by absorber lime residues. Otherwise major leaks may occur at this point which will
be detected during the self-test.
The heater’s contact surface (approx. 5 x 4 cm) for indirect heating of respiratory gas is
located immediately under the built-in compact breathing system. The heater is permanently
on during operation. In case of a malfunction, a spring-loaded contact switch causes the
heater to be immediately switched off. The heater electronics is mounted immediately under
the contact surface. The maximum heating temperature is set such that the respiratory gas
temperature at the Y-piece does not exceed 30 °C.
Note:
After switching on the master gas switch, compressed air is delivered via a filter to the
pressure regulator which reduces the gas pressure to a constant value of 1.5 bar
(backpressure). The gas is delivered to Dos2 and Dos4 (0.5 L/min). Dosage Dos4 vents the
gas flow to the PEEP valve via the 2.0 L/min dosage and the solenoid valve MY8. A pressure
of 160 mbar is generated for the roller diaphragm by means of 2.0 L dosage and dosage
Dos2. The safety valve 250 mbar discharges at a pressure of approx. 250 mbar. The gas is
delivered to the double roller diaphragm of the piston cylinder unit via the solenoid valve MY9
at the interface C1.

The solenoid valve MY8 is activated electrically and pneumatically closed. No additional gas
flows into the control system (pressure free); consequently no control pressures can be built
up for the valves.
The solenoid valve MY8 is de-energized, thus being switched so that gas can pass through.
The gas flows to the solenoid valves MY9, MY1, MY3, MY2 and MY4 (as described under
Section 5.3.1 „Device de-energized”) .
IPPV Inspiration
Valve Y1 is supplied with pressure via solenoid valve MY1 thus blocking the fresh-gas flow
inside the system. The motor drive of the piston cylinder unit sets the piston in motion. The
content of the cylinder is pressed into the compact breathing system. Fresh gas is delivered
to the patient via the absorber and the inspiratory valve. Gas is also present at valve Y7. The
blocking effect of the expiratory valve is created by a PEEP valve which is set to 40 mbar. The
piston pressure is measured at connection B1 by the pressure transducer PE2. PE1
measures the inspiratory pressure of the patient. Inspiratory O2 measurement is carried out
by the usual oxygen sensor.

At the beginning of the expiration, valve Y1 is vented via solenoid valve MY1. Gas will flow
from the compact breathing system into the breathing bag during the first moments, since
the pressure in the compact breathing system is often higher than in the breathing bag. At

The inspiration sequence is the same as described in „IPPV Inspiration”. The PEEP valve is
set to 40 mbar.
The PEEP valve switches over to the desired pressure at the beginning of the expiration
phase. The PEEP pressure is present only in the patient and in the device up to the PEEP
valve or inspiratory valve. The pressure in the piston space is less than 2 mbar at the end of
the expiration phase. Should gas loss occur in the patient area due to a sudden leakage, it is
no longer possible to build up the PEEP pressure for this breathing stroke. In the event that
a great amount of gas is lost, the message „Fresh gas low“ will be displayed during the next
breathing stroke. A deviation between PEEP actual value and PEEP target value will occur in
the PEEP function as a result of tolerances in the electrical and pneumatic elements. This
deviation is compensated by means of a software control loop. A target/actual-value
comparison is carried out after each breathing stroke in order to correct the next stroke
adequately.

Should a pressure of approx. 2 mbar build up at PE2 at the end of the expiration phase, this
pressure is recorded during the first ventilation stroke. During the next stroke, valve Y1 opens

Spontaneous breathing of the patient is made possible by setting the APL valve to „SPONT“.

Safety Valve
Should a pressure build up in the system which is higher than the control pressure, the
diaphragm in the valve Y7 is pushed away from the crater. The gas is released via the non-
return valve.
Emergency Venting
The POP-OFF valve is used as an emergency venting device. If the POP-OFF valve is
pressed, the 87-mbar control pressure will be instantly vented. As a result thereof, the
emergency venting Y7 opens. The gas in the compact breathing system flows to the
anaesthetic gas scavenging line via the non-return valve.

This message is displayed, if during expiration phase in the mechanical ventilation mode, the
piston moves to the vacuum pressure limit. Vacuum pressure means -3 mbar at P1 or -8 mbar
at P2. The message is displayed once per respiratory phase. It is accompanied by an audible
alarm if there is no connection to PM 8050 cd.
In test block 8 (IPPV leak test) this message may result from three different conditions:
• A pressure of approx. 30 mbar is generated during the leak test. This pressure is
measured by P1 and stored as reference pressure. If, during the leak test, the pressure
at PE1 exceeds the reference pressure + 10 mbar, the message “fresh gas closed” will
be displayed. If the unit is equipped with a measuring gas return line, this message will
also be displayed if the Andros bench of the PM 8050 cd is performing a calibration
during the leak test. Explanation: The ambient air drawn for the calibration flows into the
breathing system of Divan via the measuring gas return line. This leads to an increased
pressure and the message „fresh gas closed“ is displayed.
• If the pressures between PE1 and PE2 deviate by more than 10 mbar, the message
„Fresh gas closed“ is displayed instead of „P-sensor error“.
• If a relatively large leak has occured then the message „fresh gas closed“ may also be
displayed if more than half of the piston stroke is necessary to maintain the pressure.
This message is displayed if the test pressure can not be generated during a leak test before
half of the piston stroke has been carried out. It is accompanied by an audible alarm and

must be confirmed.

This message is displayed if the VT setting at the front panel has been changed to values
< 200 mL. The message is displayed only once when reaching the < 200 mL range. It must
be confirmed and is accompanied by an audible alarm.
A test stroke is activated every 10 min during mechanical ventilation, in order to check
subsystem 1 for leakage. (Subsystem 1: ventilation hoses, patient connection , inspiratory
valve). At he end of the inspiratory pause there is a pause of approx. 1 s during which the
pressure drop in subsystem 1 is measured. The message is displayed if the pressure drop is
> 15%. The maximum test pressure is 30 mbar. The message is accompanied by an audible
alarm and must be confirmed. If the leak is > 500 mL/min during the leak test and the user
accepts the leak then this message will be suppressed during operation. The message
„subsystem 1“ can also be displayed after the leak test in the self-test routine.

Pressure Limit
This message is displayed if , during inspiration in the mechanical ventilation mode, Divan
detects a pressure P1 which is > the Pmax adjusted on the front panel. The message is
displayed once per ventilation stroke. It is not accompanied by an audible alarm. (For special
characteristics refer to "P-Sensor Error")
This message is displayed if Divan detects that the pressure P1 at the end of the expiration is
10 mbar higher than the set PEEP value. The message is displayed once per ventilation
stroke and is accompanied by an audible alarm if there is no connection to the PM 8050 cd.

This message is displayed if, during inspiration in the mechanical ventilation mode, a
pressure difference of > 10 mbar is detected between P1 and P2. A set PEEP is taken into
consideration. The current piston stroke is interrupted immediately and the expiraton is
activated. In order to avoid errant messages due to sudden pressure peaks caused by the
patient, the message „Pressure limit“ is displayed after first occurrence of this fault. The
message „P-Sensor Error“ is displayed with every succeeding continuously occurring error.
The message is displayed once per ventilation stroke and is accompanied by an audible
alarm if there is no connection to PM 8050 cd.
This message is displayed if, during operation, the control pressure P3 at the 87-mbar safety
valve and the control pressure P4 at the dosage Dos3 fall below 40 mbar. (Refer to Note).
The message remains on the display until one of the control pressures exceeds 40 mbar. It
is not displayed in the Standby mode. The message is accompanied by an audible alarm if
there is no connection to PM 8050 cd.
Note
There are two ways of generating a control pressure. In normal operating mode, the control
pressure is generated via that 87-mbar safety valve (control pressure P3). Apart from this,
the control pressure is generated during the self-test or in case of failure by switching from
MY4 to Dos3 (control pressure P4).
The self-test checks whether the piston cylinder unit was inserted correctly. For this
purpose, the piston generates a small stroke starting at the removal position. If a low vacuum
is generated by this piston stroke, then the device assumes that the piston cylinder unit was
inserted correctly. If not, the message „remove piston“ will be displayed. The piston will
return to its initial position, after removing the piston and confirming the message.
It is not possible to generate a vacuum by the small piston stroke if the absorber container
has not been mounted or if the inspiratory valve plate has not been inserted (major leak). The
message „leak in breathing system“ is displayed. After confirming the message, the piston
moves to a position where it can be removed easily. The message „remove piston“ is then
displayed.
This message is displayed if a leak test was carried out successfully. The message must be

Central Electronics
The principal printed circuit board of the system electronics is the Motherboard PCB.
The CPU Standard 2 PBC and the CPU Monitoring PBC are the two brain cells of the
ventilator and use the concept of mutual data checking.
The CPU Standard 2 PCB, in conjunction with the output/input board ADDA generates the
majority of the control and actual-value signals for the peripherals such as motor control,
valve control and Front Panel PCB. Almost all the control signals are used to produce a quit
(acknowledgement) signal in the corresponding peripheral hardware. This quit
(acknowledgement) signal is then read in parallel by the ADDA PCB and CPU Monitoring
PCB using parallel ports. The analog signals for actual values and pressures are likewise
picked up by both the ADDA PCB and the CPU Monitoring PCB. They do this independently
of one another with A/D converters.
In constrast to the ADDA PCB and the CPU Standard 2 PCB, the independent CPU
Monitoring PCB is responsible almost exclusively for the check function described above
and constantly exchanges data with the CPU Standard 2 PCB via communication ports for
the purpose of verifiying the quit and analog signals.
The Motherboard PCB is located on a base plate which also accomodates the solenoid
valves for the valve controller of the compact breathing system, the electronic PEEP valve and
other pneumatic elements. Also located on the Motherboard PCB are the pressure sensors
for the airway, piston and roller diaphragm pressures, the rechargeable battery and the horn.
Peripheral Electronics
The remaining peripherals, including the motor, are connected to the motherboard by the
wiring harness. The Front Panel PCB is connected up to the motherboard by a ribbon cable.

The Motherboard PCB accomodates the three printed circuit boards: ADDA, CPU
Monitoring and CPU Standard 2.
Motor Controller
The task of the motor controller is to effect movement of the piston in the piston cylinder unit
with the correct time and amplitute by the motor and the gears. The control variable is the
position of the piston. The target value for the piston position is an output of the ADDA PCB
as the motor target value. The „travel“ is defined by impulses which are measured by the shaft
encoder. The Incremental Encoder PCB determines, from these impulses, the actual piston
position value (motor actual value) as an analog voltage.
PEEP Controller
The PEEP pressure in the expiratory airway is generated by a PEEP valve which is supplied
with a variable control pressure between 0 and 40 mbar. A plunger-coil system, an essential
part of the PEEP control pressure potentiometer, is driven by a current source, namely the
PEEP controller. The target current corresponds to the analog value, PEEP-target, specified
by the ADDA PCB. This is adjusted in the controller to the actual current, PEEP-actual.
The task of the supply voltage monitor is to constantly compare all operating voltages (+5 V, +
15 V, - 15 V, + 24 V) to corresponding ranges of voltage deviations. If a supply voltage
deviates from the given tolerance, the signals for emergency off, horn and motor
disconnection are immediately generated.

Valve Controller
The solenoid valves can only be switched if the EMERGENCY OFF signal is not active and

thus the voltage supply is guaranteed. All eight possible valves are enabled by transistor
stages by signals. The signal acknowledgements are tapped at the collectors of the
switching transistors via single-state transistor amplifiers.
The horn can be actuated by the ADDA PCB, the Monitoring PCB and by the supply voltage
monitor. It is supplied with power from the + 15V voltage or from the 9 V rechargeable
battery.
The driver ICs for RXD/DCD/TXD/DTR signals are found on the Motherboard PCB. The
signals RXD and TXD are generated by the CPU Standard 2 PCB, while DCD and DTR
originate on the ADDA PCB. The charging circuit for the 9 V rechargeable battery is likewise
located on the Motherboard PCB. It ensures that the rechargeable battery is charged
correctly.
Fan Controller
In the case of a failure in the compressed air supply, Divan will operate on oxygen. The fan
inhibits any oxygen leaking into the electronic area of Divan. The function of the fan is tested.
Opening the upper cover of the Divan reveals a right-hand cover plate under which three
PCBs are located. All three are of the plug-in type, with the left one being the CPU
Monitoring PCB.
The task of the CPU Monitoring PCB is to make sure the anaesthesia ventilator is functioning
properly. For this purpose, all signals relevant to equipment safety - both analog and digital -
are read in via 2 PIOs and an 8-bit A/D converter and evaluated by the microprocessor by
various digital control lines. The microprocessor is capable of influencing the equipment
hardware, in each case, so as to make for a safe equipment status in the event of a fault.
The CPU 6802 of CPU Monitoring PCB works indepedently and in parallel with the ADDA
PCB/CPU Standard 2; it is however connected with the latter for the purposes of exchanging
information.
System errors and faults are detected by mutual checking of the measurement and control
data and result in shutdown.
The PCB-internal hardware is also checked and faults are recognized as long as the
microprocessor itself is not shut down. In such a situation, a watchdog - a monitoring logic - is
activated and generates not only a system reset, but also various emergency-off signals

which shut down the motor and the valves and switch the Divan to Man./Spont.
The emergency-off signals and the horn switch-on are also generated by the software in the
event of a general fault.

ADDA PCB

The name of this PCB is the abbreviation of the terms „A/D, D/A converter“. Several parallel
I/O ports are located on the PCB in addition to these elements. The signals supplied by the
sensors and controllers are converted on the ADDA PCB in such a manner that they can be
processed and evaluated by the microprocessor of the CPU Standard 2 PCB. The PCB also
serves to switch the valves, normalize the pressure signal of the airway pressure sensors for
the analog output and to generate the target values for the motor and PEEP controllers.
The Front Panel PCB is located behind the touch-sensitive controls/display unit and serves
as the main board for the Display and Alphanumeric PCBs. This assembly forms the interface
between keypad and display (Display PCB, LP Alphanumeric) and the microprocessor on
CPU Standard 2 PCB. The data coming from the processor is transferred to the Display
PCB in such a manner that it can be indicated on the 7-segment LED displays and on the
bargraph. Furthermore, the control signals for the alphanumeric display are generated here.
The key matrix of the touch-sensitive controls is made accessible to the processor by a
keypad encoder. The Front Panel PCB also features the actuator and the driver stages for the
key LEDs, as well as the evaluation circuitry for the incremental encoder.
The Incremental Encoder PCB is located - viewed from the front - behind the piston cylinder
unit in the area of the drive unit. It is mounted on the right-hand carrier plate above the drive
belt. The Pulse Generator PCB is located immediately at the motor of the drive and carries
out control functions with an incremental encoder. The Incremental Encoder PCB is used to
transfer the signals received from the incremental encoder attached to the piston drive unit
and serves to determine the position of the piston. The piston receives the absolute position
during the power-on test. The Incremental Encoder PCB supplies the output signal for the
actual value of the piston position.

There are four measurement channels. Two channels are used for the conventional
measurement using an O2 sensor capsule. One of the remaining two channels is provided for
inspiratory and expiratory O2 measurement in the sidestream (intake, optical measurement
bench). Different amplification factors (v = 23.5 or v = 11.75) allow a multiplexer to detect any
short circuit between the two measurement channels. The measurement cell provides a
voltage between 9.3 mV and 21.6 mV at 21% by vol. and a voltage between 45 mV and 105
mV at 100% by volume. A sensor test is carried out after starting the unit. For this purpose, a
cell is loaded via a resistor (1kOhm) and the response of the second cell is checked
(Teledyne test).
Temperature measurement
An NTC sensor is used to measure the respiratory gas temperature. This sensor has the
following specifications:
The signals of the NTCs are processed via a temperature hybrid. It provides an output voltage
(1.4333 V at 30°C and 0.7535 V at 41°C) which is supplied to a multiplexer.
Pressure measurement
The pressure module is located on the motherboard. It generates a pressure signal which is
transmitted to a multiplexer on the Measured Value PCB. The pressure module provides a
linear output voltage between 0.110 V and 2.450 V for a pressure range between -20 mbar
and 100 mbar. The operating voltage of the pressure module is +5 V. The pressure module
consists of a differential pressure sensor which measures against ambient air. This
compensates for potential drifts. DrägerService performs a zero calibration at 6-month
intervals during the inspection procedure.

A Spirolog sensor is used for measurement of the flow. It works according to the principle of
the hot-wire flowmeter. The sensor forms a bridge between two resistors R94 and R86. The
operation amplifier (OP) N7 forms a closed-loop control system with the transistor V17 which
keeps the temperature of the hot branch at a constant level (125 °C at a flow = 0). If heat of
the hot wire is dissipated by a certain flow, the resistance in the hot wire will change. This will
detune the jumper. As a result, the control increases the current and compensates for the
heat loss.
A working resistor with a narrow tolerance is located in the hot branch of the bridge. The
voltage decrease in this branch (caused by the heating current) is a non-linear measure for
the flow. Amplified by a factor of 4, the voltage is used to adjust the sensor adjustment;
amplified by a factor of 2 it is used as a measurement signal for the flow. The cold branch of
the bridge is used for temperature compensation. The Spirolog sensor is subject to
production tolerances and ageing tolerances and must therefore be calibrated. Calibration is
carried out with a D/A converter which is controlled via a microprocessor. The D/A converter
applies a voltage to the resistor R94 which is proportional to the measurement voltage. This
resistor is connected in parallel with the cold wire and makes sure the total resistance in the
cold branch is variable. Impurities on the platinum wires can be eliminated by annealing the
wires. Annealing is achieved by detuning the jumper in the cold branch such that a high
limited current is allowed to flow through the wires. A N2O compensation can be performed
by a zero calibration with air.

The anaesthetic gases available as well as messages such as „Vapor not present“ or „Vapor
not identified“ are listed in the „anaesth. gas“ column. Only three Vapors (halothane,
enflurane, isoflurane) are presently identified automatically. Once a specific Vapor has been
identified automatically, it is no longer possible to change the anaesthetic manually. Question
marks in the display prompt the user to calibrate or to carry out a ventilator test.
The mode of operation of Divan is transmitted every 3 seconds and each time the operating
mode is changed. This allows PM 8050 cd to identify which alarm limits have to be activated.
When changing from IPPV to Man/Spont, for example, the lower pressure monitoring
(disconnection), the upper O2 monitoring and the lower anaesthetic concentration
monitoring are automatically deactivated. These alarm limits are reactivated when changing
from Man/Spont to IPPV. In both cases the changes are indicated by a flashing mode
display and by an audible alarm. No acknowledgement is required.

Each unit generates its own power failure alarm. If the power fails for a longer period, the
alarm remains active for a maximum of 30 seconds (± 10 seconds). A new power failure alarm
will be generated only after removing the cause for the first power failure. This can be
achieved by switching the main power switch ON/OFF, or if the system voltage is available
again.
• the main power switch is switched OFF/ON, or if a power failure lasts more than 120
seconds (± 5 seconds). In this case, PM 8050 cd and Divan will start the boot and the
self-test routine.
• the power failure lasts less than 120 seconds (± 5 seconds). The most recent screen
content is displayed after a warm start. The picture setup on PM 8050 cd starts approx.
20 seconds after that of Divan.
Different conditions may occur after a power failure as both units have tolerances in the
power-on behaviour.
PM 8050 cd detects that the communication to Divan has been interrupted. It tries to re-
establish communication and to log onto Divan again. If Divan is ready-to-receive,
communication will be established during a cold start. In this case, it is not necessary to
calibrate the sensors. After completion or interruption of the Divan self-test, Divan will
synchronize the modes of operation of the two units.

After a cold start or warm start, Divan suppresses the „communication interrupted“ message
for 60 seconds. After approx. PM 8050 cd logs onto Divan after approx. 30 to 50 seconds.
Divan transmits its current mode of operation to PM 8050 cd thereby interrupting the power-
on test of the monitor. The sensor do not need to be calibrated.

The optical measurement system (Andros bench 4610) consists of the measurement bench
and the relevant electronics. Furthermore, a pump module is required for delivery of the
sample gas, while a changeover valve is required for zeroing.

The sample gas (e.g. 200 mL/min) is drawn by the pump of the Andros bench. The gas flows
through a T-piece with filter and the sampling hose to the connection of the water trap. From
there it flows into a filter unit. The major portion of the gas (180 mL/mins) flows through the
Goretex diaphragm which removes the humidity. This gas portion is supplied to the cuvette
and is measured there. The remaining 20 mL/min are used to transport the collected liquid
into specific containers.
The advisory message „CO2 Line” on PM 8050 cd points out that the filter unit might be
contaminated. In this case, it is necesssary to replace the filter unit (see Instructions for Use
under „Replacing the filter unit”).

Measurement: The pump module supplies the sample gas to the measurement bench
(switchable from 60 to 200 mL/min). The gas is analyzed by using the infrared absorption
technique. For this purpose, different filters (reference filter, anasthetic gas filter, N2O filter,
CO2 filter and black filters) are positioned in the beam path by a filter wheel.

The associated electronics uses these signals to generate the values for CO2, N2O and the
anaesthetic gases (halothane, enflurane, isoflurane, as well as desflurane and sevoflurane).
These values are transmitted for each single ventilation to the I/O PCB via the RS-232
interface. From the I/O PCB, they are then transmitted to the CPU PCB. To achieve an
accurate CO2 measurement, it is necessary to perform an O2 correction. This is achieved by

transferring the O2 values of PM 8050 cd to the measurement bench. Only +12V are
required as voltage supply.
The power supply unit provides the necessary supply voltages for PM 8050 cd:

The PM 8050 cd power supply unit is electrically isolated from the Cato power supply. If the
operating voltage of the Cato changes (mains voltage), no changes must be implemented in
the power supply unit of PM 8050 cd. The power supply unit can be exchanged as a
complete assembly.
The CPU PCB is the host computer of the PM 8050 cd and performs the central tasks. The
PCB provides the bus for communication with the other PCBs. The CPU PCB accomodates
one Motorola 68000 microprocessor which can be switched between 8 and 16 MHz. The
memory consists of a 256-kB battery-backed RAM, a 256-kB unbacked RAM and a 2-MB
EPROM. Furthermore, the PCB contains a real-time clock with integrated battery-backed
2K x 8-bit RAM.
The Measured Value PCB has the task to measure the signals „airway pressure, expiratory
flow, inspiratory and expiratory oxygen concentration, and respiratory gas temperature“ and
to use them to compute and monitor important parameters (e.g. Peak, PEEP, Plat, Vt, AMV). It
consists of the measured value computer and the circuit elements for determination of the
parameters mentioned above. The printing module is not located on the Measured Value
PCB but on the motherboard. The Measured Value PCB has a Z-80 microprocessor which
communicates with the CPU PCB via a dual port RAM (1k x 8 bit). The Z-80 calculates the
flow, the pressure, the temperature, and the O2 values. The values are made available to the
Z-80 via a multiplexer, a Sample&Hold level, and a downstream A/D converter.
The SPO2 PCB is not part of the basic equipment but is available as an option. It determines
the parameters oxygen saturation and pulse rate. It communicates the I/O PCB via a serial
interface.

The Front PCB is responsible for the screen output, the generation of acoustic and optical
alarms, as well as for keyboard scanning. The PCB accomodates a graphics processor
(TMS 34010) which drives the EL display (EL = electroluminescent).
The I/O PCB is used for communication of the CPU PCB with peripheral devices such as
optical measurement bench, Divan (via Cicero PCB), SpO2 module, two serial interfaces and
one analog interface. The analog interface and the external RS-232 interfaces are electrically
isolated (2.5 kV) from the remaining equipment electronics through optocouplers. In
addition, the I/O PCB provides the functions for horn control, battery charge, temperature
monitoring and +5 V monitoring (digital).
PM 8050 cd communicates with Divan via the RS 232 interface. The modes of operation, the
settings, the compliance and the alarm messages are transmitted in co-ordination with Divan.
The electrical signal isolation of 2.5 kV from the equipment and other interfaces is ensured
by optocouplers. The voltage for the interface is supplied from Divan. If PM 8050 cd is
connected to Divan, then the contacts X5 pin 8 and pin 15 are bridged. A MOSFET (see
circuit diagram of Cicero PCB) is blocked. The main power switch is connected in series with
the internal power switch of PM 8050 cd. Since the internal power switch is always switched
on , PM 8050 cd can only be switched on/off via the main power switch. In the event of a
power failure, the position of this switch determines whether the power failure logic of the
I/O PCB trips an alarm. If both units are not interconnected the bridge between the contacts
X5 pin 8 and pin 15 is not available. The MOSFET becomes conductive via the voltage
TVBAT 2 thus causing the internal power switch to become active. In the event of a power
failure, its switching condition determines whether the power failure logic of the I/O PCB
trips an alarm. The three reflection light barriers are clocked with the signal VAPOUT. The
Vapor logic identifies the type of Vapor and transmits this information to the CPU PCB via the
I/O PCB.
Display
The display is mounted on the front frame. It is an EL display (EL = electroluminescent). The
respective electronics board is connected with the EL display. The display consists of a
640 x 400 electrode array. Light is emitted if an electrode is triggered with ac voltage via a
lines/columns combination. The electronics board provides the power supply, high voltage
driver and controller modules. The operating voltages for the display are + 5 V and + 12 V.

In the anaesthesia systems Cato and Cicero, the fresh gas flows to the patient through a
fresh gas hose and through the patient system. The A-cone is an additional fresh-gas outlet.
The A-cone and the fresh-gas outlet to the patient system is changed over via a valve.

The changeover valve is toggled between the positions “patient system” and “A-cone” using
the button. If the A-cone mode is selected, the button lamp will come on. The actual position
of the changeover valve is determined by a proximity switch.
If a warm start of the Cato or Cicero occurs, e.g. due to a short-term power interruption, the
changeover valve will remain in the position selected last. In the event of a cold start, the
changeover valve will always select the position “patient system”. If the Divan switches over to
“IPPV”, the changeover valve will also select the position “patient system”.
The switching logic and the changeover valve control are located on the A-cone PCB. The A-
cone PCB also generates the 24 V operating voltage required for switching the changeover
valve.
The operating voltage of the A-cone PCB is 5 V. It is available at the Divan/Monitor cable
harness.

The O2 sensor must be replaced if the „use-by“ date has expired or O2 calibration is no
longer possible.
• Remove the O2 sensor housing from the anaesthesia ventilator.
• Unscrew the O2 sensor housing.
• Remove the O2 sensor.
Note:
The cooling air filter must be cleaned or replaced every 4 weeks. It must be replaced
minimally after 1 year.
• Remove the cooling air filter from the mount.
• Replace the cooling air filter or wash it in warm water with a small amount of detergent.
Dry well.
• Insert the cooling air filter into the mount avoiding folds.

The NiCd battery ensures that the acoustic alarm sounds in the event of a power failure. The
battery must be replaced every 3 years.

The piston cylinder unit should be checked and serviced at regular intervals since the roller
diaphragm may become leaky as a result of ageing and frequent sterilization. The steps to
replace the roller diaphragm half on the patient side are as follows:
Step 1:
Remove the piston cylinder unit from the ventilator while in the standby mode.
Step 2:
Release the piston cylinder unit, pull straight up and remove.
Step 3:
Loosen the 2 quick-release locks (1) with a 6-mm wrench and remove the cylinder head (2).
Step 4:
Grasp the piston and unscrew the central piston screw using the 6-mm wrench and remove
the upper part of the piston (3).
Step 5:
Replace the roller diaphragm half (4) on the patient side against a new one. To ensure a tight
seal, the embossed manufacturing clock must point towards the upper part of the piston (3).
Step 6:
Use the reverse order for reassembly. Make sure neither kinks nor folds are formed during
installation of the roller diaphragm. The complete roller diaphragm must form a typical U-fold
at both ends.
Step 7:
Carry out the Cato power-on test to check the function and the tight seal of the roller
diaphragm. The roller diaphragm must unroll without forming folds.
Note: It is not advisable that the roller diaphragm half on the equipment side, ie. the non-
contaminated part, be replaced by the user. It should rather be replaced by DrägerService
since its safe function can only be ensured if various screw locking agents and special tools
(tightening torques) are used.
