Definition
1) According to elision, nursing audit refers to assessment of the quality of clinical
nursing
2) According to Goster Welfare
a. Nursing audit is an exercise to find out whether good nursing practices are
followed
b. The audit is a means by which nurses themselves can define standards
from their point of view and describe the actual practice of nursing

Lean Six Sigma focuses on eliminating defects. In healthcare, a defect can be the
difference between life and death. Lean Six Sigma to improve patient safety by
eliminating life-threatening errors.
Six Sigma healthcare projects currently underway in a few locations have already begun
to reduce the number of defects in specific areas, including the nurse charting process
and patient throughput in radiology. But there are many other patient care processes
that seem to be begging for immediate attention and initiation of a Six Sigma review. It’s
conceivable and reasonable to expect that the error rates cited in the IOM report could
be reduced by process implementation in the following areas:
1) Medication administration
2) Site-marking for surgical or other procedures
3) Assignment of patient caregivers
4) Case management
5) Patient falls
6) Patient restraints
7) Emergency department triage
