ACQUISITION REQUEST FORM

You are required to urgently submit by January 26 th 2015, a listing of all West
Indian/Caribbeana books and material [printed and e-books] which you would like
to add to your library’s collection.

The following should be taken into consideration:

 Please use the table below when compiling the listing.

 There should be two separate tables [a FICTION and NON-FICTION table]
when listing the West Indian/ Caribbeana material your library requires.
 Special emphasis should be placed on materials specific to your library and
the catchment area you serve.
 Also include material which patrons have been requesting, but are not
presently held by the library.
 Please ensure each column is completed.
 Extend the Non-fiction or Fiction tables to add as many titles as needed.
