I, IMELDA DILAG LUNA, of legal age, Filipino, widow and with
residence address at 160 Brgy. Sta. Theresa, Alfonso, Cavite, after having been
sworn to in accordance with law, hereby depose and state, that:

1. I am presently a resident of the above stated- address;

2. I declared that I have presently no source of income being unemployed

and with no business whatsoever;

3. That I am not filing an Income Tax Return (ITR);

4. I am executing this Affidavit to not for any unlawful purpose but for
applying scholarship of my Daughter CATHERINE JOY D. LUNA
with the Rizal Technological University (RTU), Pasig Campus.
