JOINT-AFFIDAVIT

WE, Jose L. Gardoce and Cherrylyn P. Canoza, Filipinos, both of legal

age and with the residential addresses at 120 General A. Ricarte St., Brgy.
San Juan, Cainta Rizal and Blk 25 Lot 3 Kamias St. Valley View Executive
Subdivision Phase 3, Cainta Rizal respectively and with cellular phone

sworn in accordance with law, hereby depose and state that:

1. I’ am Cherrylyn P. Canoza a Certified Public Account in public practice

with PRC License No. 123630 issued on _______ and with the PRC –
BOA Certificate Number 07-000106 issued on March 12, 2019.

2. That in the course of my practice as an independent auditor, I was

engaged by Avotech Corporation to perform an audit for its Financial
Statements for the period ending December 31, 2019;

3. I, Jose L. Gardoce, am the Authorized Managing Officer of Avotech

Corporation, a construction firm with office address at 120 General A.
Ricarte St., Brgy. San Juan, Cainta Rizal.

4. That Avotech Corporation is a holder of Contractor's License No.

5. That we are jointly executing this affidavit in support of Avotech

Corporation’s on-line renewal of PCAB Accreditation;

6. That we attest to the existence and due execution of Financial

Statements (with Notes to Financial Statements) for the taxable period
ending December 31, 2019 of Avotech Corporation, dated/approved on
April 1, 2020 by the board/management.

7. I, Cherrylyn P. Canoza, have audited the said Financial Statements and

issued an Unqualified Opinion that the assertions contained therein are
 fair and reasonable and are in accordance with the prevailing acceptable
standards of accounting.

8. That I, Jose L. Gardoce, states that the Audited Financial Statements

referred to above by the Auditor was made the basis in the submission of
the Annual Income Tax Return and that the same will be/was submitted
to the Bureau of Internal Revenue without any necessary corrections
and/or amendments.

9. Thus, we hereby jointly swear that the same Financial Statements with
Notes and Independent Auditor’s Opinion was made the basis for the
on-line entries for the renewal of accreditation with the Philippine
Contractors Association Board. Further that the on-line application was
performed under our joint supervision, control and responsibility;

10. That we have examined and validated every entry and information
supplied in the on-line application with the above Audited Financial
Statements and we hereby confirm the same to be true and correct and
should the Board require the submission of the Audited Financial
Statements, a copy thereof consisting of thirteen (13) pages will be
submitted.

11. We are fully aware that:

a. Any discovered misrepresentation of information encoded or

attached as a document and/or manifestations of fraud on
the application documents submitted by my firm applicant
or its Authorized Representative/Agent/Liaison Officer shall
be subjected to investigation.

b. Unconfirmed information/documents submitted to support

my firm's qualifications shall be excluded for
categorization/classification purpose; and

c. The evaluation of the firm’s qualifications shall be solely

based on the documents submitted at the time the
application was filed/accepted by PCAB.
12.WE further certify that we prepared this document that we fully
understood every word of it and its meaning, and have affixed my
signature here under voluntarily and freely with the full and complete
knowledge of the meaning and intent of this document as well as of my
rights under existing laws.
