AFFIDAVIT OF INSERTION
WE, DANILO , RAMIRO S. BACLAS, of legal age, Filipino Citizen, married, and with
residence & postal address at Blk 6 Lot 16 Golden City, Salawag, Dasmarinas City, Cavite, in
accordance with law, hereby depose and say:

That I am the representative of my daughter, MICHELLE ALBARECE BACLAS, of legal

age, single, Filipino and residing at the same address;

That they executed a Deed of Absolute Sale notarized by ATTY. GENER C. SANSAET
with Doc. No. 6545, Page No. 1306, Book 2, Series of 2017, between JUANITA ANIMAS
FERIDO and my daughter MICHELLE ALBARECE BACLAS, covered under TCT No.
T-518973;

That in the said document executed, the civil status of my daughter MICHELLE
ALBARECE BACLAS was not encoded, whereas her true and correct civil status is
“SINGLE”;

Through this affidavit, I hereby declare under oath that my true and correct civil status is
“SINGLE” as aforementioned above and request this statement to be inserted or declared in
the DEED OF ABSOLUTE SALE mentioned above;

That we are executing this affidavit to attest to the truth of the foregoing for all legal intents
and purposes.
