PETITION

PETITIONER Damie Jane A. Atoc, unto this Honorable Court,

most respectfully alleges:

1. Petitioner is 24 years old, born on March 15, 1996. She is a

Filipino Citizen, single, with residence and office address at
Block 4, Lot 2, Phase 1, Ilang-ilang street, Camella Homes, Brgy.
Pajac, Cebu City, Cebu, Philippines, with phone number:
09239635894;

2. She now applies to be given a Notarial Commission. In the

event, however, that the Petitioner ceases to maintain the
aforesaid place of business, she hereby undertakes to notify
this Honorable Court within ten (10) days from such change of
business address;

3. She has never been charged or convicted of any crime

involving moral turpitude, and herewith attached is a clearance
from the Regional Trial Court Cebu City as ANNEX “A” of this
Petition dated February 1, 2021 with official receipt number
9258347 attached as ANNEX “A-1”;

4. She has never been charged or convicted of any crime
involving moral turpitude in the Municipal Trial Court,
evidence of which is a clearance issued by the Municipal Trial
Court of Cebu City attached as ANNEX “B” dated February 1,
2021 with official receipt number 3592186 attached as ANNEX
“B-1”;

5. Moreover, a Clearance was issued by the National Bureau of

Investigation (NBI) dated December 2, 2020 herein attached as
ANNEX “C” proving that petitioner has never been charged or
convicted of any crime.

6. She is a member of the Philippine Bar with Roll Number 76207,

as evidenced by Certificate of Membership in the Philippine
Bar dated January 15, 2021, a copy of which is hereto as
ANNEX “D” and made an integral part hereof;

7. She has paid her privilege taxes for the year 2021 as evidenced
by Professional Tax Receipt No. 9325674, a copy of which is
attached hereto and made an integral part hereof as ANNEX
“E”;

8. She is a member of good standing of the Integrated Bar of the

Philippines, as evidenced by a Clearance from the Integrated
Bar of the Philippines Cebu Chapter and for whom two (2)
executive officers of the Integrated Bar of the Philippines Cebu
Chapter issued a Joint Certification of her being a member of
good standing and of her good moral character, which Joint
Certification dated January 15, 2021 is attached as ANNEX “F”.
Official receipt thereof numbered 86245 is attached as ANNEX
“F-1”;

9. She has no pending administrative case before the Office of the

Bar Confidant of which a copy of the Certification is attached
hereto as ANNEX “G”;

10. Three (3) un-retouched passport size-colored photographs

of Petitioner, taken within thirty (30) days before the filing of
this Petition, with her signature at the bottom, is attached
herewith as ANNEX “I”;

 11. Petitioner’s signature appears on the sheet of paper
attached hereto and made an integral part hereof as ANNEX
“J”;

12. Petitioner’s proof of filing and payment of this petition is

attached as ANNEX “K”; and

13. Petitioner has not simultaneously applied for commission in

any other court of the Philippines.

PRAYER
WHEREFORE, it is hereby respectfully prayed that Petitioner be
granted authority to purchase seal and be granted as a Notary Public
for and in the City of Cebu City Philippines within the jurisdiction of
this Honorable Court and that Petitioner be for year 2021 until
December 31, 2022.

address at Block 4, Lot 2, Phase 1, Ilang-ilang street, Camella Homes,
Brgy. Pajac, Cebu City, Cebu, Philippines, after having been sworn to an
oath in accordance with the law, depose and state:

1. That I am the Petitioner of the above entitled proceedings that

prepared the foregoing petition;

2. That I have reviewed the Petition and the allegations therein are
true and correct based on authentic records;

3. Further, I certify that I have not filed any other petition for
issuance of notarial commission in any other court or tribunal in
the Philippines.
