AFFIDAVIT /WAIVER

I, PRESCO C. MALAZA, Filipino, of legal age, married, with residence and postal
address at Poblacion, San Isidro, Surigao del Norte, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am not the owner of TWENTY SEVEN (27) SQUARE METERS of a

Residential Lot under in my Parcel which is more particularly described as follows:

A PARCEL OF RESIDENTIAL LOT, located at Poblacion, San Isidro, Surigao del

Norte, declared in the name PRESCO MALAZA with an area of 623.42 sq.m., under
Tax Declaration No. 13-20-0100642, series of 2014, under Cadastral Lot No. 164,
with an assessed value of P46, 760.0, with the following boundaries:

NORTH: LOT NO. 158/160/163/037/033/028 SOUTH: LOT NO. 266/Sec.17

EAST: LOT NO. National Road/033 WEST: LOT NO. 261/260/039/040

2. That Twenty Seven (27) square meters portion of the said Residential Lot
was erroneously obtain by me and included in this parcel.

3. That in view of this, considering that it is only Five Hundred Ninety Six Point
Forty Two (596.42) sq. meters PORTION of this parcel is mine and Twenty Seven
(27) square meters PORTION is for GINALYN M. TORMENTO. I herby waived,
relinquished and returned the TWENTY SEVEN (27) square meters PORTION of the
said parcel above-mentioned to the true owner GINALYN M. TORMENTO.
