Re: Henry Bastian, age: 53-years-old

Dear Ms. Kopelowit,

I am writing to request regular follow-up and home visits for Mr. Bastian, a 53- year- old . He
was under our care since the past few days following a fall from the staircase injuring his left
knee. After continous dressing, the wound has healed well however, he still requires assistance
when walking or making any movements, please note that he has been asked to walk using
walking sticks. He is due for discharge today and will soon be transferred under your care.

On the morning of 10th april, Mr. Bastian rumbled down the stairs injuring his left knee and was
taken to a local general practitioner immidiately. The practitioner then suggested his wife,
Kendra the wound will require dressing on a regular basis and will have difficulty in
performing activities such as taking shower and readily recommended Athlone health care
agency for the wound dressing and providing assitance when taking showers everyday. After 3
days when the dressing was changed, the wound looked healthy with no signs of infection and
was able to make movements with assistance from his wife, Kendra. Nonetheless, he does
complain of pain when making movements which is normal in such cases. Gradually, after 4 days
his knee was re-assessed and the wound was all healed and is planned for discharge.

Worthy to note, 2 years back the patient was diagnosed with CVA and since then he developed
speech defect along with problems with his body posture and balance. For this reason, he has a
slightly slurred speech and uses a walking aid for mobility.

Looking at the patient's social environment, Mr. Bastian is a married man living with his wife,
Kendra. She has no helper at home and takes care of her husband by herself alone.

In view of the above circumstances, on the behalf of Mrs Bastian, I request you to kindly
arrange someonewho can help her by making regular home visits so that her husband can
achieve a normal life as soon as possible. i look forward to your kindness that would warmly
accept Mr. Bastian into ypur care.
