 In the morning at hospital X in Denpasar area, there was a patient who had just been
hospitalized due to shortness of breath or dyspnea. A nurse will do a physical exam.

A. PRE-INTERACTION PHASE
1. The nurse is mentally prepared and confident.
2. The nurse has understood about shortness of breath or dyspnea.
3. The nurse has obtained patient data.

B. ORIENTATION STAGE
Nurse : “Good morning”
Patient : “Good morning nurse”
Nurse : “my name is Sindy, I am a student from STIKes Wira Medika Bali,
on duty this morning. If I may know what is sister name?
Patient : “My name is Ristika”
Nurse : "Sister can help me tell the date of birth"
Patient : “15 July 2000”
Nurse : “Okay Sis, I came here for a physical examination”
Patient : “Yes Nurse”
Nurse : “Before I do my physical exam, do you have anything to ask?”
Patient : “No nurse”
Nurse : “Then I will start physical examination, yes”
Patient : “Yes Nurse”

C. WORKING STAGE
Nurse : “You can sleep on your back, okay?”
Patient : “Okay nurse”
Nurse : “I start from the head yes,
Head: clean hair, no wounds, no bumps
Eyes: not anemic
Nose: not a secret
Mouth: mucosa is moist, not difficult to swallow
 Neck: no lumps, and lymph nodes are not enlarged
Chest: there are additional voices
I've finished the physical exam!”
Patient : “What are the results of my nurse's physical examination?”
Nurse : “In the chest that is having problems”
Patient : “Then what should I do brother?”
Nurse : “Later the doctor will explain the next steps”
Patient : “Good nurse”

D. TERMINATION STAGE
Nurse : “Ok sis, I have done the action, how did you feel after i did the physical
exam?”
Patient : “I feel more comfortable”
Nurse : “Ok sis, thank you because earlier sister wanted to follow the instructions
I gave!”
Patient : “Ok nurse”
Nurse : “Before I leave, does anyone want a sister to ask?”

Patient : “No nurse”

Nurse : “Okay then I'll go back to the room, good morning”

Patient : “Good morning”
