DEED OF DONATION

KNOW ALL MEN BY THESE PRESENTS:

THIS DEED of DONATION, made and executed by, EDNA R. ROQUE , of legal age,
married, Filipino Citizen, and with residing at Block 16 Lot 20 Amethist St. Diamond Village, Salawag,
Dasmarinas City, Cavite, hereinafter called the DONOR, in favor of his REX C. RAQUIN, herein
after the DONEES;

WITNESSETH

That the DONOR is a plan holder of ST. PETER with plan type ST. SAN FRANCIS and life
plan Agreement with LPA NO. N16623449D, duly issued by the outherment of St. Peter Life Plan, Inc;

That the DONEE is the brother of the DONOR;

That FOR AND IN CONSIDERATION of the DONOR’S love and affection to the DONNE and
as act of gratitude and liberality on her part, the DONOR hereby voluntarily and hereby give
TRANSFERS, CONVEYS by way of DONATION unto the said DONEE the abovementioned ST.
PETER LIFE PLAN for the used of late REX C. RAQUIN, who was died on March 21, 2020, in St.
Peter Kalibo Aklan;

That the DONOR that this donation is not made with intent to deceive their and that they have
reserved for themselves sufficient fund and property;

The DONEES do hereby accept receive this donation made in her favor by the DONOR and
hereby manifest her gratefulness for the latter generosity, gratitude;

IN WITNESS WHEREOF, we have hereunto set our hands this 3 rd day of September 2020, at
Dasmarinas City, Cavite, Philippines.
