2. Outline of breakdown
To use the ballast pump before entering an overseas port on November 25, an attempt to start the No. 2
Diesel Generator was carried out by way of remote control, but it did not start due to a bad automatic
starting valve. When it was started at the engine side, water gushed out from the No. 6 cylinder safety
valve. The engine was stopped immediately and inspection of each part was carried out. It was found
that cooling water was leaking from the mating face of the exhaust valve casing jacket and the seat at
one location for each of the No. 4 and No. 6 cylinders. (See Fig.)
It is presumed that the leaked water entered the cylinder, and from there entered the suction pipe and
exhaust pipe, and eventually to the other cylinders.
3. Repair
4. Cause of breakdown
The No. 2 Diesel Generator exhaust valve had a water leakage from a section linking the exhaust
valve casing and cylinder cover for cooling water. To prevent it from becoming mixed with lock arm
LO, it had been replaced with a maintained spare parts about 10 days before. However, it is presumed
that two of the parts were insufficiently maintained.
