AFFIDAVIT/AUTHORIZATION FOR LETTER OF ADMINISTRATION/SUCCESSION CERTIFICATE

oath that legal heirs mentioned below have authorized undersigned to act on their behalf for the
purpose of filling the application for the grant of succession certificate/Letter of Administration in

establishment of NADRA.
