INTRODUCTION

BACKGROUND

Medical tourism refers to people traveling abroad to obtain medical treatment. In the past, this
usually referred to those who traveled from less-developed countries to major medical centers
in highly developed countries for treatment unavailable at home. However, in recent years it may
equally refer to those from developed countries who travel to developing countries for lower-
priced medical treatments. The motivation may be also for medical services unavailable or non-
licensed in the home country.

Over the years, India has grown to become a top-notch destination for medical value travel
because it scores high over a range of factors that determines the overall quality of care. Imagine
a complex surgical procedure being done in a world class global hospital by acclaimed medical
specialists at a fifth to tenth of what it normally takes! That’s India. From quality of therapy, range
of procedural and treatment options, infrastructure and skilled manpower to perform any
medical procedure with zero waiting time, the list of benefits of travelling for medical treatment
in India are many.

India has not only hospitals with world-class facilities but skilled world-class doctors and medical
personnel too. The country has the largest pool of doctors and paramedics in South Asia (1.2
million Allopathic doctors. 0.17 million dental surgeons, 2 million nurses). Many of them have
established their credentials as leaders around the world. India’s medical history spans thousands
of years through many medicine forms. With a large number of doctors, there is a high level of
competency and capability in adoption of newer technologies and innovation and fresh
treatment methods.

The project proposes a medical tourism mainly aimed at patients in need of post trauma
rehabilitation. Trauma is a mainly caused from unintentional injuries, homicide. Injuries
associated with trauma account for approximately one third of all emergency department visits
and 8% of all hospital stays in trauma care systems. The top four mechanisms of traumatic injuries
involve falls, motor vehicle accidents (struck by or against another individual), and other
transportation accidents. The mechanism of injury varies in different age groups. For those
younger than 7 years or 75 and older, falls account for 40% of traumatic injuries; but in those
aged 15–33 years, motor vehicle injuries account for 27% of the injuries, peaking around age 19.
A trauma center is a hospital that has the resources and equipment to help care for severely
injured patients.

Injury is an increasingly significant health problem throughout the world. Every day, 16000
people die from injuries, and for every person who dies, several thousand more are injured, many
of them with permanent sequelae. Injury accounts for 16% of the global burden of disease. The
burden of death and disability from injury is especially notable in low- and middle-income
countries.

Not only at the time of accident, rehabilitation is an important part of the trauma care, there is a
vast amount of preventable injury-related disability, especially due to fractures and burns of the
extremities. Efforts to prevent such disabilities are needed in acute care . Efforts are also , there
needed in the rehabilitation of people with injuries after the acute treatment phase is over, to
maximize recovery of independent function. Hence, basic physiotherapy/occupational therapy
for those recovering from extremity injuries (especially fractures and burns) is deemed essential
at all hospital levels. This includes such activities as the proper use of splints to prevent burn
wound contractures, and range-of-motion and strengthening exercises for recovery from all
types of extremity injuries. In the light of the fact that many injured patients receive follow-up
care at basic facilities after hospital discharge, such capabilities are desirable at this level of
facilities. Obviously, it would be ideal to have fully trained physiotherapists and occupational
therapists providing such care at all levels. However, given limitations of cost, appropriate
elements of training in physiotherapy/occupational therapy might reasonably be provided to key
personnel. The fields of physiotherapy and occupational therapy overlap somewhat and vary
between countries. For the purposes of these guidelines, physiotherapy refers to those services
needed to improve range of motion, strength and mobility. Occupational therapy refers to those
services needed to improve range of motion and strength, specifically for the upper extremities,
and to assist patients in regaining independent function for tasks such as self-care (e.g. dressing,
feeding). The latter includes the provision of adaptive devices and training in their use. The full
spectrum of physiotherapy, including that appropriate for patients with injuries of the head and
spinal cord, is deemed desirable at specialist and tertiary care hospitals. The full spectrum of
occupational therapy is deemed desirable at specialist and tertiary care hospitals. As indicated
above, the provision of fully trained professionals in each field is ideal.

AIM
The project aims to provide an enabling infrastructure for the people in need of post trauma
care in such a way that it creates a better environment for those suffering from post trauma
problems, in a way which will address the growth in the path of medical tourism.

OBJECTIVE
• To set up the country’s first ever rehabilitation centre concentrating only on post \\
• trauma care.
• Research to promote the importance of post trauma care.
• Exploration of factors that can create an environment that lay positive impact on health
outcomes of the people suffering from post trauma problems.
• Development of a block to promote the research and study of trauma care.

RATIONALE
Road deaths in India increased to nearly 1.49 lakh in 2018. In 2017 the country had registered a
3% decrease in road death. Roads are considered a sign of development bringing colossal benefits
to community as socioeconomic and logistic facilitator. Yet, growth of road network has brought
road crashes leading to civic pain from premature deaths of productive age group (wage-earning
and child-raising group). Safety on road is a vital issue of well-being of any country. In India, two-
thirds of road traffic injury (RTI) deaths are reported in the age group 15–44 years. In 2017,
officially reported road accidents were 464,910, claiming 147,913 deaths and 470,975 injured
persons, that is, 405 deaths and 1,290 injuries each day from 1,274 accidents. In the absence of
trauma registry, adding unreported accidents, it is frightening to note this fairly high rate when
compared with developed economies.RTIs are fatal or nonfatal trauma from road crashes
concerning at least one moving vehicle. Children and elderly, pedestrians, and cyclists are the
most common victims on road. The experts at the Academy of Family Physicians of India
visualized public health components of road safety that extrapolates beyond the compulsory use
of seat belts and helmets. Therefore, a committee of translational researchers from the related
disciplines was formed for the systemic situational analysis followed by needful
recommendations.

Post-traumatic disorder is found in a person who had faced a life threatening event or a
victim of severe trauma.fractures,burns are major injury related disabilities, stress can also
be caused,During a life threatening event, a victim may lose control over what is happening
and feels to be in extremely dangerous condition. This effect may cause deep abnormality
in the individual sense within and may lose control over themselves and self-consciousness.
Individuals who have faced or been a victim to a traumatic event that include physical or
sexual violence, a serious accident, an earthquake, flood or other nature disa sters, have a
higher risk of developing post traumatic stress disorder. After the event, the individual may
feel scared and confused and may disrupt the life, making it difficult to continue with even
routine activities.

So especially in a city like Bangalore which has ranked among the top cities having high rate of
road accidents a trauma care resort is gonna be highly effective and with the help of medical
techniques the medical tourism can also be supported. Even though hospitals are present near
the madugurkhi village one that takes care of the post trauma patients is absent. Figure showing

SCOPE
The project proposal envisages presenting a fully equipped trauma rehabiltation centre of
excellence and medical tourism zone for patients needing post trauma care to which people from
around the world will be attracted for availing the best system of treatment supported and
supplemented by modern scientific methods, making use of the expertise of eminent doctors in
medical field and benchmark the project as an integrated centre of clinical excellence.

LIMITATIONS
The project only concentrates on the field of post trauma care and none of the others fields.

LOCATION DETAILS
• The subject is located at Devenahalli in northern Bangalore a rapidly growing logistics and
software hub with the International airport already in operation acting as a fuel for the growth.

• The residential catchments of Yellhanka, Vidyaranyapura,Sahakaranagar Hebbal, Jakkur,


Hennur road are all excellently connected and within a short distance from the subject site.

• These micro markets constitute a major section of the HR pool in the north Bangalore
• The subject land parcel is off the Bellary Road NH& connecting Bangalore and Hyderabad which
is also well connected to the entire city by means of New airport road, elevated corridor up to
the BIAL and also well connected to the rest of Karnataka. The time taken for travel at any point
of time during the day between CBD and the site is not more than 45 minutes.

• Spread over an area of 20 acres the subject site has an access to the national highway from all
regional and local transport networks this property has a significant location advantage in the
north Bangalore sub market.

Rehabilitation services can provide help and care to assist with people having a poor physical
state after severe trauma.. Rehabilitation services aim to increase the independence and physical
function of the people after injury and to return you to as much of your previous ability as
possible. Where it is not possible for you to return to doing things as you used to, the
rehabilitation process will help you explore new ways of doing things.

Depending on your injury, rehabilitation may helps to regain movement and strength, relearn
skills or learn new ones. It can also help you to recover mentally and emotionally from the
trauma you have experienced.

Often trauma injuries can cause even states of getting paralysed hence it might be very difficult
for rest of the family members to spend their quality time looking after the injured ones ,in
such cases trauma rehab centres play a very important role.

ARCHITECTURAL RELEVANCE

Numerous studies show evidence of the body’s ability to “self-heal” when put into positive
healing environments. This healing is enabled by the ability of the body to ‘tap into our internal
pharmacies’ by activating the body’s powerful neurochemicals such as endorphins. Spaces are
good mediums for affecting emotions. In places where intense emotions run high, materials,
sounds, smells, light, and colour have an enormous influence on how people perceive themselves
and cope with their situations. In healing spaces, the patient’s experience is often undervalued,
overlooked by the functions and operations of the medical treatments and busy schedules.

Healthcare buildings are to be designed as living spaces for patients rather than warehouses for
the sick. It has to be remembered that a this is not a factory in which the assembly lines dictates
all aspects of design but is a community in which the patient is fundamental to the successful
working of the whole. Needs and expectations of the patients have to be visualized, analyzed and
fulfilled. The healing spaces should provide the patients a sense of safety, comfort, dignity and
repose. It should also provide pleasing spaces for patients, families and visitors as well as imbibe
the cultural concerns of the community.

The ability of architecture to heal the mental and physical illness with the different planning
parameters and the elements that can enhance healing has been explored. Without
compromising on the need, the ability of all patients to be outdoors in a good environment
is, in my view, the absolutely greatest strength in the design of this caring environment.

• the connection to Self: Healing begins when one is in a space of retrospection. The space
however is one that must be comfortable and a reflection of oneself.

• The connection with the City: The city environment is harsh and busy and tends not to
offer many places or spaces of retrospection and contemplation. The site should be a part
of the city but detached from the fast pace. The end-user should be able to arrive
conveniently with public transport to the site to allow a large catchment of people seeking
a place for healing. The site should also be in a place that is not difficult to navigate and
find.

• The connection to Nature: Human beings cannot live without natural surroundings. The
symbiotic relationship that exists between the two is one that has to be nurtured. A
physical and visual link with nature has positive beneficial qualities that help in healing
and health in general.

• The connection with Light: Natural sunlight kills harmful bacteria and cleans air. Apart
from these obvious natural benefits of sunlight, natural light - when controlled - gives a
space character that is sensual so giving a space an emotive quality which can be felt by
the end-user. Light is also a symbol of growth and life as it benefits the natural world in
that way. This study on ‘Healing spaces in architecture’ is an endeavor of understanding
the conceptual way to achieve healing. Since it is vast field of study with the number of
ongoing researches, this has profuse scope for further investigation.
