Premium Rates:
The annual premium rates for this Plan are as set out below. The premiums payable for this Policy will be calculated in
accordance with the rates specified in the premium rate table. Please note that the premium rates for GlobalCare Health
Plan are not guaranteed. These rates may be adjusted based on future experience. The Company may, at its sole discretion,
vary the premium rates by giving You one (1) month prior written notice. The annual premium is based on the Life Assured’s
Age on his birthday nearest to the Policy’s commencement or anniversary.

Important Provisions:
The following are some key provisions found in the Policy. This is only a brief summary and You are advised to refer to the
actual terms and conditions in the Policy. Please consult Your AXA Financial Planner/Adviser or Your FA representative if You
require further explanation.
1. Eligibility
(i) Life Assured to be eligible for cover under this Policy must be aged eighty (80) years or less at the time of insurance
application. The Policy can be renewed each year till the Life Assured reaches the age of ninety nine (99) years, subject
to the payment of the applicable premium.
(ii) Life Assured below the age of eighteen (18) years must have one of the parents as Policyholder.
(iii) This Policy may provide cover for Life Assured residing outside of Singapore as long as the Policy is purchased in
Singapore. However, in some cases We may not be able to cover You if You reside in certain countries due to local
regulations or other limitations.
A newborn infant (born to an insured mother) can enjoy cover commencing at the time of birth provided We are requested
to cover that infant within 30 days from the time of birth and insured mother’s Policy is in force at the time of delivery. If the
mother is not insured under the GlobalCare Health Plan at the time of delivery a newborn baby may only be covered and be
eligible for benefit after final discharge of the child into parental care.
2. Free-Look Period
We give You a period of fourteen (14) days from the date You receive this Policy to review it. If Your Policy is delivered by post,
it is considered to have been received by You seven (7) days from the date of posting. If You decide to cancel this Policy, You
must write to Us and return the Policy documents and membership card within the period of 14 days allowed. We will refund
the premium paid less any expenses incurred in processing Your application provided that no claims have been made during
this period.
3. Termination
You may apply to Us to terminate the Policy by sending Us a written request on Our prescribed form. When We accept Your
application for termination of Policy, the Policy will be terminated on the date We receive the notice of termination.
The Policy will terminate at the earliest occurrence of any of the following events:
a. Incorrect information, statements or descriptions supplied by the Policyholder or Life Assured, or if the Policyholder or the
Life Assured breaks any of the terms of the Policy or makes, or attempts to make, any dishonest claim; or
b. The premium is not paid within the Grace Period; or
c. The Policyholder or the Company terminates the Policy; or
d. The Life Assured dies; or
e. The Policy Anniversary on or after Life Assured’s ninety-ninth (99th) birthday; or
The termination of the Policy will be without prejudice to any claims arising prior to the termination.
4. Refund of Premium
Where refund of premium is applicable, We will refund fifty percent (50%) of the annual premium paid for the current period
of insurance.
Refund of premium, if any, is given only when the premium is paid on annual mode and provided that:
(i) as at the date on which the notice of termination is received by Us, the Policy has been in force for less than six (6)
months in that year; and
(ii) no claim is made in that year.
Any premium paid on or after the date on which this Policy is voided or revoked shall be refunded to You without interest. In
the case of fraud, there will be no premium refund.
5. Renewal
This Policy is automatically renewed on each Policy Anniversary by payment of the renewal premium before the premium due
date provided that Your Plan is still available for renewal. On the Policy Anniversary We may vary the terms and conditions,
benefits, cover and/or premium or even cancel all policies in a particular age group of a Plan type by giving You thirty (30)
days advance notice in writing. However We will not cancel Your Policy simply as a result of your personal claims. In the event
that We are required by law to make a change during the Period of Insurance, We will be obliged to do so before the next
Policy Anniversary.
Cover under this Policy will cease automatically on the Policy Anniversary if We do not receive the premium due within the
Grace Period. This will happen whether or not written notice of cancellation has been given by Us to You.
Non-renewal of this Policy will not affect claims related to eligible Treatment given before the Policy Anniversary.
The payment to or acceptance by Us or Our agent of any premium subsequent to the termination of this Policy will not create
any liability on Our part except that of refunding any such premium.
6. Non-Guaranteed Premium
Please note that premium rates for GlobalCare Health Plan are not guaranteed. These rates may be adjusted at the time of
renewal based on future experience. The Company may, at its sole discretion, vary the premium rates by giving one (1) month
prior written notice.
7. Change of Principal Country of Residence
In the event the Life Assured changes his/her Principal Country of Residence, and even if he/she is staying in the same
Area of Cover, the Policyholder must inform Us within thirty (30) days of such change. If the Policyholder fails to notify Us, We
reserve the right not to pay any benefits.

8. Change in Benefits
You may apply to Us to change the Plan type of the Policy by sending Us a written request on Our prescribed form. Such
application must be made at least thirty (30) days prior to any Policy Anniversary, provided that:
(a) the Plan type applied for is still available at the time of Your application;
(b) the Life Assured is within the age limit for that Plan type;
(c) for a change to a Plan type with higher benefits, You furnish Us with satisfactory evidence of the insurability of
the Life Assured at Your own expense, which must be acceptable to Us; and
(d) You pay any additional premiums applicable for the new Plan type.
If the new Plan type has lower benefits, We will not refund any part of the previously paid in respect of the higher benefits
under the former Plan type.
The new Plan type will take effect from the next Policy Anniversary.
Any condition known about or that should reasonably be known about at the time of an amendment or upgrade must be
advised to Us before the Policy amendment takes effect. In the event that We do accept a request for an upgrade, We may
restrict cover for conditions existing at the time of the upgrade to the level of benefits enjoyed under the original Policy.
9. Reinstatement
Policy reinstatement is not allowed for all GlobalCare Health Plans. The Policy will terminate if the premium due is not
paid within the Grace Period. (A period of 30 days from the due date for any premium payment made, except for the first
premium)
What is not covered by the Policy:
There are certain conditions under which no benefits will be payable under the Policy. These are stated as exclusions under
the contract. Please check the Policy for details. The following are some of the more important exclusions:
(i) Treatment of any pre-existing condition and associated medical conditions unless allowed for by Your Plan and as per
rules described in Clause 7 – ‘Pre-existing conditions and moratorium underwriting’.
(ii) Investigations into and treatment of infertility, contraception, assisted reproduction, sterilization (or its reversal) or any
consequence of any of them or of any treatment for them;
(iii) Treatment of impotence or any consequence of it
(iv) Treatment of sexually transmitted diseases
(v) Sex change including treatment which arises from or is directly or indirectly made necessary by a sex change
(vi) Treatment of any medical condition which arises in any way from HIV infection
(vii) Treatment of obesity or any medical condition which arises from, or is related to, obesity in any way including but not
limited to the use of gastric banding or stapling, the removal of fat or surplus tissue from any part of the body whether
or not it is needed for medical or psychological reasons
(viii) The costs of collecting donor organs for transplant surgery or any administration costs involved even if such transplants
are allowed by the terms of this plan
(ix) Treatment which arises from or is directly or indirectly caused by a deliberately self-inflicted injury or an attempt at
suicide. GlobalCare Health Plan will not pay Death Benefit if Life Assured dies by suicide within one (1) year from the
commencement date
(x) Treatment which arises from or is in any way connected with alcohol abuse or drug or substance abuse
(xi) Treatment directed towards developmental delay in children whether physical or psychological or learning difficulties
(xii) Preventive treatment contohnya: vitamin, supplement, general checkup, health screening, vaksinasi
(xiii) The costs of providing or fitting any external prosthesis or appliance
(xiv) Treatment of injuries sustained from playing professional sport or from base jumping, cliff diving, flying in an unlicensed
aircraft or as a learner, martial arts, free climbing, mountaineering with or without ropes, scuba diving to a depth of
more than 10 meters, trekking to a height of over 2,500 meters, bungee jumping, canyoning, hangliding, paragliding
or microlighting, parachuting, potholing, skiing off piste or any other winter sports activity carried out off piste
(xv) Any charges made by medical practitioner, hospital, laboratory or any such medical services which are not Reasonable
and Customary (R&C)
(xvi) Any charges for treatment related to and/or the correction of congenital conditions and/or deformities whether or not
manifest and/or diagnosed or known about at birth
