RISK 1 : SHIP - IN CASE BRAKE MECHANISM FAILS, MAY LEAD TO LOOSING THE LIFEBOAT, SERIOUS INJURY TO PERSONNEL INSIDE
LIFEBOAT OR DAMAGE TO THE LIFEBOAT ITSELF. UNCONTROLLED DESCENT OF THE BOAT INTO WATER, IMPACT SHOCK LOADS.

Instruction
1) Every crew member to be instructed to take the lifeboat on
load before removing the pin from brake.
2) Ensure brake is set in position and all aligned at all times.
3) A responsible officer to be present at all times during the
operation for active supervision.
4) Maintenance procedures followed as per maker instructions.
1) If any doubt refer to Lifeboat manual.Crew to be aware for the
Launching and release mechanism.
2) Don't allow unauthorised person nearby during operation.
3) Use of proper PPE is mandatory.
4) Proper communication is to be maintained at all times.

5) Marking on brake system to be adhered to before and after the
operation.
6) Dry run of Lifeboat to be performed without crew. Lifeboat to
be lowered and heaved up 2 times to check everything is in
working order before conducting the actual exercise.

meeting carried out prior operation.

RISK 2 : SHIP - EQUIPMENT FAILURE AND/OR MALFUNCTION - FAILURE OF THE FALL WIRES. FAILURE OF ONE OF THE WIRES WOULD
CAUSE THE BOAT TO TIP. PEOPLE WOULD FALL FROM THE BOAT. BOAT WOULD SWING ON ONE FALL AND COULD IMPACT WITH SHIP.
