Herewith my resume, this letter of intent indicates my interest and willingness to apply as
NURSE DEPLOYMENT at DOH REGION 11. I am currently working as
a HEMODIALYSIS NURSE with JO STATUS on MAGUINDANAO PROVINCIAL
HOSPITAL under INTEGRATED PROVINCIAL HEALTH OFFICE –
MAGUINDANAO.

I have received a degree in BACHELOR OF SCIENCE in NURSING (BSN) from NOTRE

DAME UNIVERSITY. I graduated last 2015 and passed the licensure examination that same
year, and got a board rating exam rating of 80.80%. I am currently on my last year for my
MASTERS of ARTS IN NURSING MAJOR in CLINICAL MANAGEMENT at DAVAO
DOCTORS COLLEGE INC.

I underwent (3) three months of hands-on training in HEMODIALYSIS at LAVIE DIALYSIS

CENTER located at G/F FTC Tower 1034 Mt. Apo Street Davao City, Davao Del Sur under
PRIMUS REVIEW and TRAINING CENTER with 400 HOURS hands-on experienced on B
BRAUN DIALOG+ HEMODIALYSIS MACHINE.

Aside from that, I am a CERTIFIED IV NURSE THERAPIST with ANSAP IVT Card No.
16-001023, CERTIFIED NURSE PHLEBOTOMIST, CERTIFIED BLS & ACLS
HEALTH CARE PROVIDER of AMERICAN HEART ASSOCIATION (AHA). I also have
(1) year experience working in a DELIVERY ROOM set up at BUNTIS BIRTHING HOME
CLINIC at 028 Gonzalo Javier Street RH 7 Cotabato City.
