Nursing Considerations:
1. Question for possibility of pregnancy. Drug is contraindicated in pregnant women. It may cause fetal harm.
2. Before treatment, assess patient for underlying causes of hypercholesterolemia and obtain a baseline lipid profile and hepatic profile. Obtain periodic LFT results
and lipid levels before starting treatment and at 6 and 12 weeks after initiation, or after increase in dosage and periodically thereafter.
3. Obtain dietary history. Educate the patient that he should follow a standard cholesterol-lowering diet before and during therapy. Include teachings about weight
control, exercise, and importance of controlling high-fat levels.
4. Monitor for headache. Assess for rash, pruritus, malaise. Watch for signs of myositis and myopathy (unexplained muscle pain, tenderness, weakness, malaise, dark
urine, fever). Drug may be discontinued.
5. Avoid excessive alcohol intake and large quantities of grapefruit juice.
Why was it given to the patient?
Atorvastatin was given to reduce LDL levels of the patient and to prevent cardiovascular complications, such as stroke or myocardial infarction, for which the patient
is in high risk of having secondary to her diabetes mellitus and chronic kidney disease.
