AFFIDAVIT OF EXPLANATION

I, MARILOU B. ESTORES, of legal age, Filipino, single and a resident of

Biac na Bato Silangan 1 4106 Rosario, Cavite, Philippines after having been duly
sworn to in accordance with law, hereby depose and state that:

1. That I am the mother of Rommar B. Estores who was born on November

3, 1994 at Mandaluyong City;

2. That due to my negligence and/or inadvertence and the prevailing

conditions during then, I failed to register his birth certificate immediately;

3. That I am executing this Affidavit to attest to the truth of the foregoing

facts and for whatever purpose it may serve.
