AFFIDAVIT OF LOSS
I, JON JON MANGULABNAN y MERCADO, Filipino, of legal age,
and a resident of 31 Alasas, City of San Fernando, Pampanga, after being
duly sworn in accordance with law, depose and state that:

1. I am worker of Gardenia Philippines with the position of Salesman:

2. Relevant to my position, I am in charge of our products that are

evidenced by Sales Invoices (S.I.);

3. On 30 July 2020, I noticed that S.I. P001327325, which was supposed

to be under my custody, was missing;

4. Despite diligent search and efforts to locate the said invoice, I could
not find the same, such that I believe that it is now lost beyond
recovery;

5. As such, I am executing this Affidavit of Loss to attest to the truth of

the foregoing and for any legal purpose it may serve.
