    COMMON MEDICAL ABBREVIATIONS

    -c - with ac - before meals


    Tx - treatment pc - after meals
    Dx - diagnosis hs - bedtime
    Rx - prescription WA - while awake
    Fx or # - fracture NOC - night shift
    Thx - therapy ad lib - as desired
    Sx - surgery EOS - end of shift
    d/t - due to +/-  - with or without
    s/t - secondary to CP - chest pain
    r/t - related to SOB - shortness of breath
    r/o - rule out N/V/D - nausea, vomiting,
    c/o - complains of diarrhea
    - change Afib - atrial fibrillation
    VSS - vital signs stable GIB - GI bleed
    Abx - antibiotics CHF - congestive heart failure
    Q - every ARF - acute renal failure
    S/S - signs and symptoms ACS - acute coronary syndrome
    -1 WNL/WDL - within normal
    -2 limits, within defined limits
