Information & Instructions for completing an Application Form
1. The applicant CAB shall undertake to carry out its calibration activities in such a way as to meet the requirement of ISO/ IEC 17025:
2005 or ISO/IEC 17025:2017 whichever is applicable, NABL specific criteria, other relevant requirements of NABL and the regulatory
authorities, as applicable at all times. CAB applying for accreditation as per ISO/IEC 17025: 2017 shall comply with NABL document
NABL 165; 'NABL's Policies for Accreditation as per ISO/IEC 17025:2017'.
2. Applicant CABs are advised to ensure that the latest versions of NABL documents are available with them.
3. The application fee and other necessary charges related to accreditation process is given in NABL document NABL 100 'General
Information Brochure' under NABL Finance and NABL Fee Structure'. NABL 100 is available on NABL website.
4. CABs are advised to familiarize themselves with NABL 100 'General Information Brochure', NABL 201 'Procedure for dealing with
Changes in Accredited Conformity Assessment Body's Operations', NABL 216 'Procedures for Dealing with Adverse Decisions',
NABL 131 'Terms and Conditions for Obtaining and Maintaining Accreditation' and NABL 133 'NABL Policy for Use of NABL
Symbol / Claim of Accreditation by Accredited Conformity Assessment Bodies & NABL Accredited CAB Combined ILAC MRA

5. The applicant CAB shall provide photocopy of following appropriate document (s) in support of the legal status claimed:
i. Proprietorship firm (Copy of Bank Passbook/Account Statement of CAB & ID of Proprietor)
ii. Partnership (Copy of Registration under 1932 Act)
iii. Company Act (Copy of Registration under 1956 Act)
iv. Societies Registration Act (Copy of Registration under 1860 Act)
v. Indian Trust Registration Act (Copy of Registration under 1882 Act)
vi. Government (Copy of Government Notification / Declaration etc.)
6. The applicant CAB shall intimate NABL about any change in the information provided in this application such as scope applied for
accreditation, personnel, and location etc. within 15 days from the date of changes.
7. The applicant CABs must participate satisfactorily in the Proficiency Testing program conducted by APLAC or any other national or
international accredited/ recognized PT provider. In the absence of the same applicant CABs need to participate in self initiated inter
CAB comparison according to NABL-164. For participation in PT, refer NABL document NABL 163.
8. The CAB shall also inform NABL in advance about any reservation regarding appointment of Lead Assessor/ Assessor for the
assessment.
9. The applicant CAB shall be given due notice of any intended changes relating to NABL accreditation criteria and will also be given such
time, as in the opinion of NABL is reasonable to carry out the necessary adjustments to its procedure(s). The CAB shall inform NABL
when such adjustments have been completed.
10. 10. The application must be filled up carefully to provide required information in such a manner that further correspondence for seeking
clarifications are not required. Particularly the scope of accreditation shall be complete to indicate unambiguously:
a. DUC / parameter(s) / measured quantity
b. range of calibration
c. Calibration and Measurement Capability (CMC)
11. The details of CAB locations, calibrations, which the CAB intends to cover, vide NABL accreditation must be listed clearly. The
calibrations those performed at site or with a mobile facility should be clearly identified in the scope of accreditation
Note:CAB operates from different locations in the same disrtict may apply for NABL accreditation as a single CAB. The details of each
location shall be explicitly mentioned in application form. In events where the CAB operates from different district, each CAB shall apply
separately for accreditation except those cases where safety or regulatory requirements are there for operation of the CAB. In such
cases, the CAB shall provide the proper justification.
12. The CAB shall offer the NABL or its representative cooperation in:
a. undertaking any check to verify calibration capability of the CAB.
b. providing names of all authorized signatories who are responsible for authenticity and issue of calibration certificates / reports.
c. offering access to relevant areas of the CAB for witnessing the calibration being performed.
d. examination of all relevant documentation and records.
e. interaction with all relevant personnel.
13. The Calibration and Measurement Capability is expressed as the least uncertainty of measurement (ï‚±) at a defined confidence
probability level. It must be expressed at 95%. The coarser of the uncertainty claimed and the demonstrated during the on-site
assessment shall be recommended as CMC. However, in case, assessor recommends the observed uncertainty as CMC, a proper
justification should be there.
14. The CAB shall take all necessary actions and discharge all non-compliances raised during the assessment within stipulated time. The same
shall be verified to the satisfaction of NABL. The final decision on accreditation shall rest with NABL.
15. The application shall be kept confidential (until required by law) by NABL and information obtained during the processing of application,
assessment visit and grant of accreditation shall be safeguarded and dealt with impartiality. The procedure for processing of application
for accreditation is given in NABL 100.

Terms & Conditions for Obtaining and Maintaining NABL Accreditation
The terms and conditions mentioned in this document is considered as accreditationagreement between NABL and Conformity Assessment
Body (CAB). By accepting theseterms and conditions, it is implied that a CAB as an applicant and after obtainingaccreditation agrees to comply
at all times with all Terms and Conditions for NABLAccreditation. Any violation of this terms and conditions (Accreditation agreement)
shallresult in adverse decision as per NABL 216 â€˜Procedures for Dealing with Adverse Decisionsâ€™including legal action.
The Conformity Assessment Body {Testing Laboratory/ Calibration Laboratory/ MedicalTesting Laboratory/ Proficiency Testing Provider
(PTP)/ Reference Material Producer(RMP)} that are applicant or accredited by NABL shall be required to fulfill the following termsand
conditions:
1. The Conformity Assessment Body (CAB) shall carry out its Testing / Calibration / Medical/PTP / RMP activities in such a way as to
meet the requirements of ISO/IEC 17025/ ISO15189/ ISO/IEC 17043/ ISO 17034 whichever is applicable, relevant NABL
specific criteria(wherever applicable) and other policies of NABL
2. CAB shall ensure that the latest versions of NABL documents are available with them andare effectively implemented.
3. CAB shall familiarize themselves with all relevant standards, NABL documents etc.applicable for particular accreditation scheme.
4. The CAB shall have a valid legal identity as given below.
a. Proprietorship(Bank passbook/ Account statement and PAN of the CAB)
b. Limited LiabilityPartnership(Registration certificate under The Limited Liability Partnership Act,2008)
c. Company (Registration certificate under The Companies Act, 1956 or 2013)
d. Societies/ Trust(Registration certificate under Societies Registration Act, 1860/Registration under The Indian Trusts Act,
1882)
e. Government(Gazette or Government Notification or self-Declaration on Letterhead by Head of the organization)
5. It is the responsibility of CAB to abide by the National/ Regional/ State/ Local regulatoryrequirement/ Acts/ Rules/ Legal orders/
Court Decisions/ Orders issued by Government/Statutory Bodies as applicable and effective from time to time.
6. The CAB shall identify and define various activities which they are involved in, and ensurethat it does not lead to any potential conflict
of interest.
7. The CAB shall have qualified and trained personnel for applied/ accredited scope includingsampling (wherever applicable), person
declared by CAB to NABL who is responsible toreport, review and authorize the results who meet the requirements of relevant
NABL specificcriteria (wherever applicable) for each field / discipline of accreditation.
8. The CAB shall have qualified and trained personnel who is responsible for formulatingopinion and interpretation in the report/
certificate (wherever applicable).
9. Wherever applicable, the CAB shall follow the test methods as mentioned in the currentNational or International standards and as
stipulated by regulatory bodies. Where suchmethods do not exist, other validated methods are acceptable. In case CAB uses in-
housevalidated methods, the validation data should be submitted along with the application.
10. Applicant CAB shall successfully participate in at least one PT program prior to gainingaccreditation in each discipline applied.
Requirements specified in NABL 163 shall befollowed for PT participation.
11. CAB shall submit the completed application for renewal of accreditation six months prior tothe expiry of accreditation and also agree
to undergo assessment as per the scheduleproposed by NABL to maintain continuity in accreditation cycle. Application submitted
afterexpiry of accreditation will be considered as fresh application.
12. The CAB shall offer cooperation to NABL or its representative in:
a. Undergoing assessments in stipulated time intervals / whenever NABL considered it asrequired.
b. Access to all CAB areas of operations including Mobile/ temporary, site facility,subcontractor premises, wherever relevant and
applicable.
c. Undertaking any check / inspection to verify the capability of the CAB for the applied /accredited scope.
d. Witnessing the activities being performed relevant to accreditation.
e. Assessing the competence of the staff (including staff working in shift operations / at site)during assessment.
f. Access to all relevant information and documentation
g. Access to those documents that provide insight into the level of independence andimpartiality to the CAB from its related bodies,
if applicable.
h. Access to all records pertaining to relevant personnel.
i. Assessing the competence of the persons who are responsible to report, review andauthorize the results,
j. Assessing the competence of the persons who are responsible for opinion andinterpretation,
k. Unannounced visit,
l. Investigating any complaints related to NABL accreditation, against the CAB.
13. During the accreditation period, the CAB shall claim accreditation in only those premises,and scope for which it has been accredited
(as stated in Accreditation Certificate and scopeof Accreditation).
14. Accredited CAB shall adhere to â€˜NABL 133â€™ for the use of NABL symbol, use of NABLAccredited CAB Combined ILAC
MRA Mark as well as any claim of accreditation of the CAB.CAB shall ensure that it fully conforms to the requirements of NABL
and ISO/IEC 17025 /ISO 15189 / ISO/IEC 17043 / ISO 17034 for claiming accreditation status while makingreference of its
NABL accreditation in communication media. Actions like suspension, debar,legal action etc. may be taken in case of violation of
NABL 133.
15. CAB shall use NABL accredited CAB Combined ILAC MRA Mark only after obtaining writtenpermission from NABL.
16. Accredited CAB shall make it clear in all its contracts with customers that its activities fallingunder accredited scope in no way imply
that the product so tested or equipment calibratedis approved by NABL.
17. The accredited CAB is neither allowed to make misleading / unauthorized statementregarding its accreditation nor refer to
accreditation in such a way so to imply that a product,process, service, management system or person is approved by NABL.
18. The CAB shall pay application fees for accreditation, re-accreditation; expenses towardstravel, boarding & lodging for any kind of
assessment including supplementary visit, on-sitesurveillance, desktop surveillance, re-assessment and annual accreditation fees as
shall bedetermined from time to time, by NABL.
19. The CAB shall inform NABL without delay of significant changes affecting the CABâ€™sactivities and operations relevant to
accreditation, such as:
a. its legal, commercial, ownership or organizational status,
b. resources and premises,
c. scope of accreditation,
d. sub-contractor addition (RMP), and
e. other such matter that may affect the ability of the CAB to fulfill the requirements of NABLaccreditation including policies, ISO/
IEC 17025 or ISO 15189 or ISO/IEC 17043 or ISO17034 whichever is applicable.
20. The CAB shall inform NABL within 15 days of changes, such as:
a. the organization top management, key personnel including sampler (where applicable),person responsible for report, review and
authorizing the results,
b. main policies, and
c. any change in the Sample Collection Centre/ Facility (ies) to NABL.
21. The CAB shall regularly visit NABL website to keep itself updated with the latest versions ofNABL documents.
22. CAB shall always have a dedicated person responsible for implementation of ManagementSystem.
23. The CAB shall unambiguously declare the name of any individual and organization that hasprovided consultancy for preparing
towards NABL accreditation.
24. Where a CAB subcontracts a substantial or critical part of the accredited activities that workshall be placed with another accredited
CAB only.
25. The accredited CAB shall commit to comply with the changes initiated by NABL in itsaccreditation criteria, policies and procedures.
The CAB shall inform NABL when suchalterations under the agreed time frame have been completed.
26. The CAB shall take all necessary actions and discharge all non- conformities raised duringthe assessment within the stipulated time to
avoid any adverse decisions against it asdetailed in â€˜NABL 216â€™. The same shall be verified to the satisfaction of NABL. The
finaldecision on accreditation shall rest with NABL.
27. CAB shall not use its NABL accreditation in such a manner as to bring NABL into disrepute.
28. CAB shall not claim NABL accreditation after shifting premises, till communication on grantat new premises is done by NABL
29. At any point in the application or initial process, if there is an evidence of fraudulent behaviourand/ or if the CAB has intentionally
provided false information or conceals information, NABLshall reject the application or terminate the assessment process.
30. If an evidence is found of fraudulent behaviour or the CAB intentionally provides falseinformation or conceals information, NABL
shall initiate adverse action as per NABL 216.
31. The accredited CAB upon suspension, withdrawal of accreditation or debar from reapplying(however determined) shall forthwith
discontinue claim of accreditation in any form in alladvertising material and return the accreditation certificates to NABL.
32. CAB shall inform its affected clients of the suspension, scope reduction, withdrawal ofaccreditation or debar from reapplying and the
associated consequences without unduedelay.
33. The accredited CAB can relinquish accreditation by giving notice in writing to NABL bysurrendering the accreditation certificates
after clearing the outstanding amount, if any.
34. The CAB shall inform NABL, if any of the proposed assessor(s) happens to be theirConsultant or associated with the CAB in any
other capacity (personal or professional).
35. NABL absolves itself of any legal or financial liability arising out of activities of any of itsaccredited CAB involving any accidental or
consequential damages to personnel /equipment / products at any time.
36. CAB shall not offer any gifts or any kind of payments in cash or any undue favour to theassessment team members. In case of any
violation, NABL shall suspend the accreditationof the CAB as per NABL 216.
37. The application shall be kept confidential (unless required by law) by NABL and informationobtained during the processing of
application, assessment visit and grant of accreditationshall be safeguarded and dealt with impartiality.
38. CAB shall offer co-operation to NABL assessment team in carrying out unannouncedassessment as a part of continuous monitoring
activity by NABL for its accredited CABs.
39. CAB shall ensure the capability w.r.t. applied scope for accreditation (availability ofresources such as personnel, equipment, CRM,
method validation etc.) before applying toNABL.
By accepting these terms and conditions, it is implied that a CAB as an applicant and afterobtaining accreditation agrees to comply at all times
with all Terms and Conditions forObtaining and Maintaining NABL Accreditation. Any violation of this terms and conditions(arrangement) shall
result in adverse decision as per NABL 216 â€˜Procedures for Dealing withAdverse Decisionsâ€™ including legal action.
Dispute, if any, arising out of NABL decisions that remain unresolved through mechanismprovided by NABL are subject to the exclusive
jurisdiction of the Courts at New Delhi andnone other.
We declare that
1. We shall comply with the terms and conditions of maintaining accreditation (NABL 131) and will abide by them.
2. We shall fully comply with ISO/IEC 17025:2005/2017 or ISO 15189:2012 or ISO/IEC 17043:2010 or ISO 17034: 2016, which ever
is relevant for the accreditation of Conformity Assessment Body.
3. We shall comply with all national, regional and local regulatory requirements for operating a laboratory.
4. We agree to comply with accreditation procedures and pay all costs for pre-assessment, assessment, verification visit (if any), surveillance
and reassessment irrespective of the result.
5. We agree to co-operate with the assessment team appointed by NABL for examination of all relevant documents by them and their visits
to those parts of the laboratory that are part of the scope of accreditation.
6. The information provided herein with reference to the consultancy provided to us and also in respect of any adverse decision, taken by
NABL or by any other Accreditation Body is true to the best of our knowledge and believe and nothing has been concealed.
7. All other information provided in this application is true and nothing has been concealed.
8. For any payment which has not been made in response to the bills raised by NABL, we are liable for adverse action which may be
initiated by NABL as specified in NABL 216.
9. If any information is found to be false or has been concealed wilfully, we are liable for any lawful action which may be initiated by NABL
as deemed fit.
