Parent letter

Dear parent(s) of Jacoby,

Over the next few weeks I plan to work with Jacoby to improve in many areas. He is a
very bright, active and smart. He has so many wonderful qualities and skills. I have developed
four goals for Jacoby to work towards.
Goal number one is a cognitive goal. Jacoby will be at a developmentally appropriate
stage of cognitive development over the next year. To continue his development cognitively
Jacoby will continue to discuss the weeks theme for a minimum of 2 minutes per lesson with
the teacher. He will also answer a question during each activity that is checking for
understanding. Jacoby will also be working on his social skills.
Goal number 2 is an affective goal. Jacoby will attend visually to the task when
prompted by the teacher for five minutes of each activity during a 50-minute lesson. One of the
objectives I have developed to work on his social skills is to accept teacher feedback. I will give
him feedback during his performance of the skills in hopes that Jacoby will apply this feedback
to future attempts of the skill. Jacoby will also be expected to use all equipment properly and as
instructed.
Goal number 3 is a psychomotor goal. Jacoby will continue to improve his competency
In object control and motor skills by practicing for the next year. Jacoby will be given activities
from me that he can use to work on these skills. I will provide feedback during and after
activities.
Goal number 4 is a fitness goal. Jacoby will continue to remain active by participating in
MVPA at least once a day for 15 minutes minimum. I am impressed with how active and willing
Jacoby is to do all the activities provided to him. To continue to work on fitness I suggest he is
participating in moderate to vigorous physical activity (MVPA) for at least 15 minutes a day. He
can do this in many ways. Jacoby can utilize his trampoline for fitness.
Jacoby has been a pleasure to work with so far and I can’t wait to see the improvements
he makes over the next few lessons! I am excited to see accomplish some goals and objectives.
