NURSING

I. ASSESSMENT
1 . Anamnesa
Prostate only experienced by male client.

2. Main Complaint
Complaints are often experienced by clients known as LUTS (Lower urinary tract
symptoms) among others hesistensi, weak urine jet, intermitennsi, the
presence of residual urine after micturition, urgency, frequency and
dysuria.

3. Disease History Now


The patient came to the hospital with complaints of difficult urination, the amount
of urine. The process of urination lasts longer, and the patient was not satisfied at
the end of urination. The patient complains of pain during urination.

4. Formerly Disease History

In the review of the disease ever suffered by the client such as heart disease,
kidney, and hypertension, as well as surgical history ever had a good
experience when dealing with the onset of BPH, or not. (brunner & Suddart,
2001: 1629)

5. Family history of disease

In reviewing whether the client's family member is suffering from a disease such
as clients, infectious diseases such as tuberculosis, and hereditary diseases such as
diabetes, hypertension, heart disease, and asthma.
II. Review of system :
 B1 (Breath): -
 B2 (Blood):
 Pulse >100x/menit
 Blood pressure >120/80 mmHg

 B3 (Brain) : -
 B4 (Bladder) : difficult urination, urinary process lasts longer, bladder distention
 B5 (Bowel) : -
 B6 (Bone) : -
