Contract for equipment rental

This is a legally binding contract between __________________________ representing

_______________________ and Edward Lee.

Signature at the bottom expresses agreement with all the following terms.

1. Rental of below listed equipment shall be from _____ to_____(time) on __/__/__ to

__/__/__ (date)
2. Compensation to Edward Lee for this rental will be
$100 for rental of PA system consisting of one crown 1950 Watt Amplifier and 2 Ampeg
speakers.
$150 for 2 Technics turntables and a dj mixer
$15 for each Table
$3 for each folding chair.
The rates above indicate rental for an 18 hour time period. The rental period begins
when the renter signs the agreement and ends upon conclusion of check-in of
equipment. Rentals are only made for 18 hour time periods. There are no prorated
refunds given for early return of equipment.
3. Edward Lee shall not be held responsible for equipment failure during rental period.
It is the sole duty of the one renting the equipment to make sure the equipment when
picked up is in operational order. If anything should happen to the equipment during
rental the renter as identified above accepts responsibility for all costs to get equipment
repaired back to it’s original state or replace the equipment or provide a functionally
equivalent replacement.
4. If equipment is returned late the renting individual and organization accept
responsibility for billing of another 18 hour time period plus an additional $35 late fee.
5. Cancellation of event rental equals forfeiture of deposit unless notice is given two
weeks in advance, failure to notify Ed Lee of cancellation of rental within forty-eight
hours of event start time shall result in billing of the organization and or individual renting
equipment for the full amount.
6. Above mentioned company/establishment and individual will be held responsible for
damage/theft of equipment or damage caused by crowd members, faulty wiring, and so
forth. Furthermore the above named company and individual is soley responsible for
adequate care and security of the equipment from the time of rental start to time of rental
end.
7. Edward Lee is not responsible for damage to facilities from equipment, it is the
responsibility of the one renting equipment to make sure that electrical outlets are not
being overloaded, and that the sound equipment is in proper working order, and able to
accept the reasonable electrical requirements made by dj equipment. It is also the
responsibility of the renter to follow laws as related to music amplification.
8. Should the company or establishment become unable to pay Edward Lee for any
reason the representative who signed this contract with Edward Lee shall be held
personally liable for any and all costs.

12. I agree to pay any fees, or collection costs incurred by Edward Lee should my
payment be delinquent.

13 I authorize Edward Lee to charge my credit card for all costs, charges, and fees
related to my rental including the cost of any repairs or replacement of equipment.

I being of sound mind and body by my below signature state that I am of lawful age, and
legally competent to sign this affirmation and release; that I understand the terms
expressed in this document are contractual and not a mere recital; and that I have
signed this document as my own free act. This Release and indemnity agreement shall
be binding upon the registrant and me, and our heirs, personal representatives, agents,
successors and assigns. Every term on the above signed contract has been read by me
and initialed with my own hand to verify that I have read and agree with the terms
therein. I have signed this contract in the county of Queens, New York on __/__/ 20___.
