APPLICATION PROCESS (within 2 to 3 weeks)
Only e-mail applications with complete requirements will be processed. Please send these in one e-mail to
1 EnterpriseExtension@Smart.com.ph within the above-mentioned service window only. Applications with
incomplete documents will not be processed.
Any change in the plan while the application is in progress will revert its status to either credit checking or

validation.
3 This cascade will only apply to New Connect and Retention of Existing Enterprise Extension subscribers.

REQUIREMENTS FOR NEW CONNECT AND RETENTION
Commercial Document: Signed and duly accomplished Service Application Form (SAF). Below are the
1 important notes.
a. Please shade if it’s New Connect or Retention. If Retention, please indicate the Mobile Identification Number
(MIN or the existing mobile #) at the corresponding portion.
b. Subscriber may choose to have a higher handset model through either one time cash-out or amortization.
Please indicate the amount at the corresponding column.
c. Don’t forget to indicate the exact handset model (including the capacity as needed).
*Inventory Officer will automatically allocate any color based on the availability of stocks at Step 4.
d. SAF should be printed and signed properly (not “pasted”), and sent as scanned file in PDF Format. Signature
should be identical as the one reflected in the Company ID.
e. Delivery Address should be indicated if Residence or Office Address. Please refer to Step 7 under Process for
further instructions.

REQUIREMENTS FOR NEW CONNECT AND RETENTION
2 Credit Documents:
b. One month latest pay slip reflecting date hired if applicable / Certificate of Employment with Compensation
c. Good credit standing in SMART (applicable to those with existing accounts and those Enterprise Extension
subscribers due for retention)

APPLICATION PROCESS (within 2 to 3 weeks)
Credit Checking: Subscriber will be e-mailed should there be concerns on the application. SMART Enterprise
2 reserves the right to decline any application based on credit evaluation.
Commercial Validation: Validation officer creates the service request reference number. He/ She will e-mail
3 the subscriber should there be clarifications on the plan to be subscribed.
Common scenarios when the validation officer requires confirmation from the subscriber:
· Incomplete handset details – memory size/ model is not indicated in the SAF
· One-Time Cash-Out/ Amortization value is not indicated/ is not consistent with the current handset matrix
· Signature in the SAF is not consistent with the Company ID presented
Inventory: Preferred handset is allocated. There are times though that the preferred handset is out of stocks.

APPLICATION PROCESS (within 2 to 3 weeks)
Come and Pay Notice (CAP Notice): Provisioning officer will e-mail this to the subscriber once the preferred
5 handset has been allocated. This contains the advance payment to be settled – For New Connect, these are
the one month service fee and handset cash-out/first month amortization, if applicable. While for Retention,
these are the handset cash-out/first month amortization, if applicable as well.
The CAP Notice has a validity of fifteen (15) calendar days. Once subscriber has settled the amount,
payment is posted within one to three (1-3) working days, depending on the payment channel which the
transaction was made.
Non-payment of the CAP Notice within the said period will automatically cancel the application.
Delivery Preparation (1-2 days): Upon posting of payment, the SMART EE Postpaid Kit is prepared at SMART’s
6 Warehouse in Metro Manila, and then endorsed to courier WWWExpress.

To inquire the issued WWWExpress Waybill #, please call SMART Enterprise 24/7 Hotline at +632-848-8889
using a landline # or #888 using a SMART mobile number. Please be ready with the SR # indicated in the
CAP Notice as reference.

HANDSET WARRANTY
Applicable only to Non-Apple handsets: If the handset manifested an inherent defect within seven (7)
days upon receipt, this is covered by a replacement warranty c/o SMART Enterprise. Please call #888
(toll-free using your mobile) or 8848-8889 (landline). The handset (box and complete accessories) will be
arranged for pull-out. Defect/s will be validated by SMART support. Once confirmed, a new handset will
be issued to the subscriber (timeline is subject to availability of stocks).
Applicable to all handset brands: Handsets are covered with twelve (12) months manufacturer’s
warranty through its accredited service centers (depending on the handset brand) - Unit will be
repaired free of charge if due to factory defect. Warranty does not cover customer negligence
resulting to physical or internal damage and cosmetic problems. Replacement parts and other fees will
be charged accordingly; only labor is free within the first 12 months.

subscriber will present to the preferred accredited service center.
