JOINT AFFIDAVIT OF TWO DISINETERESTED PERSONS

(Re: DISCREPANCY/ONE AND THE SAME PERSON)

We, PERLA U. DANIEGA and RAYMON LAURENTE MILLET, of legal ages, Filipinos and with
residence address at 21 J. Perez St., Project 4, Brgy. Masagana, Quezon City and 7107 Phupa Compound,
Pinagbuhatan, Pasig City, respectively, after having duly sworn to in accordance with law do hereby
depose and say, that:

1. We personally know the person of MANUEL MERCADO who was born out of wed-lock on 7
December 1989 at Quezon City General Hospital in Quezon City from the Spouses GLORIA
MERCADO and biological father MANUEL BATILO (deceased);

2. Being the longtime family friends of the abovenamed, we have been known some of their
material factual circumstances, particularly of MANUEL MERCADO, the antecedents, to wit:

3. To reiterate, MANUEL MERCADO’s biological parents are the common spouses GLORIA
MERCADO and MANUEL BATILO (deceased).

4. That MANUEL MERCADO’ remains illegitimate because his biological parents never got married
until his biological father’s demise;

5. It was so happened, however, that despite his biological father, MANUEL BATILO, never
executed any instrument that will acknowledge his paternity relationship with MANUEL
MERCADO, during the latter’s tender age, he was using “MANUEL MERCADO BATILO” in his
previous scholastic records, specifically from grade school until Second-Year High School level,
but the same was corrected to MANUEL MERCADO only when he was at the Third-Year High
School, this upon learning that his registered name in his Live Birth Certificate was “MANUEL
MERCADO”.

6. Thus, from then on, he already been using MANUEL MERCADO in all his present undertakings,
such as in his latest issued NBI Clearance, PAGIBIG member’s data form, and PHILHEALTH ID
Card, except in his SOCIAL SECURITY SYSTEM’s (SSS) record where his name was erroneously
indicated as “MANUEL BATILO MERCADO” instead the correct “MANUEL MERCADO,” and based
on our personal knowledge, the same was done only out of his sheer negligence, or an honest
mistake, hence, he sought the correction of entry over the same.

7. Further, we jointly affirm and declare that “MANUEL BATILO MERCADO” as per his SSS record is
the same MANUEL MERCADO that reflects to his other government issued identification
documents such as his latest issued NBI Clearance, PAGIBIG member’s data form, and
PHILHEALTH ID Card.

8. That neither of us are related nor interested to whatever purpose this affidavit may sought for.

9. That we joint executing affidavit to attest to the truth of the foregoing instrument and for any
legal purposes this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 29 th day of March, 2021 in Quezon
